Supervision
During the visit a school age child was playing at the end of the educator's driveway with the (REDACTED). The educator could not observe or adequately supervise the child while she was inside the home with the other 5 children present. In addition, the school age supervision permission form signed by the parent only allowed for the child to play in the backyard area without direct supervision.
