The following was observed: In the MIDDLE room, there was a tear in the window screen. In the BACK HALLWAY and BACK classroom, there was chipped and peeling paint.
A review of the evacuation drill log indicated that November and December 2025 were not documented.
A review of all staff records indicated that one did not have evidence of a current physical examination.

It was observed that there was no barrier to the provider's kitchen from the BACK ROOM to the CHILDREN'S BATHROOM and that the door connecting the kitchen to the MIDDLE ROOM could not fully close, leaving the kitchen fully accessible to children (last cited on 1/11/23).
A review of three attendance records indicated that one child was not signed into the BACK ROOM's attendance, however, the Educator was aware of the number of children under their supervision. This was immediately corrected.
It was observed in the MIDDLE ROOM that the pink play kitchen was not secured and tippable.

During the visit, the staff schedule posted did not reflect minimum educator ratios at all times, including breaks.
It was observed that the sink faucet in the CHILDREN'S BATHROOM was heavily corroded and in need of repair.

It was observed that there was chipped paint accessible to children in the MIDDLE and BACK rooms.
It was observed that there was no evidence of a list of allergies or emergency medical information posted for a child who had a (REDACTED) and was prescribed medication.
It was observed that the green wire fencing around the back of the outdoor play area was not fully secured.
A review of three staff records indicated the following: One did not have evidence of a current physical examination, one did not have a current EEC PQ registration, and one did not complete the minimum number of professional development hours for the year.

Medication
The follow was observed and accessible to children: In the MIDDLE room, the plastic dramatic play set was unsecured and tippable. In the CHILDREN'S BATHROOM, the top of the metal garbage container was rusted.
A review of an Individual Health Care Plan was incomplete. It did not include the second medical treatment needed and which staff were designated to administer medication.
A non-prescribed medication indicated on an Individual Health Care Plan was not stored in the program.

The following hazards were observed indoors: the door to the kitchen was left open, leaving the area accessible to the children. The kitchen included cleaning chemicals, a purse and gray lighter. In the children's bathroom, a plastic Target shopping bag, plastic packages of pull ups and toilet paper were accessible to the children. In the rear hallway, items stored on the shelves must be secured.
A thermometer was not observed in the refrigerator used to store children's food items.
Two children with medication on the premises did not have a Medication Consent Form signed by the parent.
The following hazards were observed outdoors: There are several tree roots protruding from the ground and present a tripping hazard. A trash bag and garden hose were accessible to the children. The white gate had a broken picket with sharp edges.
In the Middle Classroom, a water bottle, thermos, and Gatorade bottle were not labeled with the children's names. Personal items intended for individual use by children, including but not limited to bottles, pacifiers, toothbrushes, and sleeping materials, must be labeled with the name of the child for whom they are intended.
An educator was observed working with the children and did not have a Background Record Check through EEC as required.
The allergy lists were not posted in a manner that protected the children's privacy. The licensee must post a list of allergies and/or other emergency medical information provided by the parent in a manner that protects the privacy of each child.

The garbage barrel in the kitchen was not covered. Garbage must be kept in lined and covered containers and all trash containers must be emptied at least daily.
