1. The educators in the PreK classroom did not take a child's emergency medication with the group to the playground. 2. One child's physical identified the use of an inhaler as needed. The program was not aware of the medical condition and did not have this medication on the premises. Therefore these medications were not immediately available if needed.

In the Toddler 1 classroom, a food bowl was observed in the handwashing sink, and a child's feeding bib was lying on the side of this sink. Facilities used for hand washing after diapering or toileting must be separate from facilities and areas used for food preparation and food service.

The furniture must be secured in the Pre-K, Toddler 1, Toddler/Preschool, and Preschool 1 classrooms to prevent them from tipping over. In the Toddler/Preschool classroom, push pins were used to hang string lights in the classroom. Push pins should not be used in classrooms with young children due to the potential for serious injury and choking hazards.
One child with a chronic medical condition did not have an Individual Health Care Plan as required.

In the Infant Classroom, a teacher was observed sitting in the rocking chair, holding a child, while scrolling through their cellphone. Educators must not engage in any other activities or tasks that could unnecessarily divert their attention from supervising the children.
In the Infant Classroom, bibs, spoons, and an empty food container were observed in the handwashing sink adjacent to the changing table. Facilities used for hand washing after diapering or toileting must be separate from facilities and areas used for food preparation and food service.
In the Awesome Owls Classroom, three bookshelves and a wooden kitchen set were not secured to prevent them from tipping. The garbage barrel did not include a cover.
One child's Individual Health Care Plan had expired on 5/31/2024.
In the Infant Classroom, a teacher's purse was observed on the floor by the children's cubbies.
