There was a bottle of the provider's medication within reach on the stairs. There were piles of laundry and clutter in the dining room, leaving no space for the SA child to sit and do activities.

There were items such as adult medications, tools, shaving products in the drawers in the dining room, bathroom and bedroom that were accessible. There were some outlets that were not covered around the home. There was a shelving unit in the hall near the dining room that was not secured. Wooden tray tables in the dining area were not secured.
There were toiletries and cleaners under the sink accessible in the downstairs bathroom. There was an outlet uncovered in the bedroom with a liquid air freshener within reach.
