Four children's records were reviewed and found they were all missing the CPR authorization from the parents and one record was missing the physical description or photo of the child.
The following was observed during the visit: The changing tables in both Woods (Coyotes/Foxes) and Grasslands (T/PS) were across the room and not adjacent to a sink for handwashing. There must be running water or an approved alternative adjacent to each diapering area.

Three staff records were reviewed and it was found one staff record was missing evidence of completion of EEC Essentials 2.0 (hired 8/2024).
The following was found to be accessible to children: In the LADYBUG bathroom, there was paint chipping on the heating vent cover and in the ART room bathroom, paint was chipping on the wall.

An infant child (over 12 months old) was observed to be sleeping on a mat in the corner of the classroom and the teacher was sitting on the floor not positioned to have direct eye sight to see the sleeping child. An additional teacher entered the room and sat so they could see the child. Educators must be positioned to maximize their ability to see and hear children in their care.
Emergency medications readily available
A child's antihistamine was not available as the family had not supplied it.
