The following was observed to be accessible to children: In the INFANT/TODDLER classroom, two spots of peeled paint were seen on the back of one of the shelves that separate the room from the Infant room. The facility must be free of chipping, flaking and peeling paint.
Two staff records were observed and found one record was missing evidence of a second MMR vaccine.

While reviewing the two staff records, it was observed that one staff member who is currently working in the program did not have a completed EEC Essentials 2.0 certificate. This educator had taken and completed EEC Essentials 1 and had begun 2.0 but had not completed by the time of the visit.
During the visit, the attendance in the Toddler/Preschool classroom was not accurate. The attendance reflected four children were present but there were five children in the classroom. This was corrected during the visit. Program must maintain accurate attendance, staff child ratio, and appropriate group size.
One child's IHCP was not current (expired 7/2025).
One child's medication consent form was not current (expired 7/2025).

When the Toddler/Preschool Classroom transitioned back into the building, one child in that group remained outside with their Early Intervention worker.

Full staff records were not provided as requested by the date this visit was published. Only the Staff Records Checklist was provided.
The following was observed to be accessible to children: -In INFANT/TODDLER, the kitchen set was not secure and tippable; In TODDLER/PRESCHOOL, the bookshelf below the castle bulletin board was not secure and tippable and the kitchen set has one screw exposed in the back where it was previously secured to the wall (another screw is secured in the wall); and the top electrical outlet in the hallway near the PS/SA room did not have a cover.
The Infant Classroom does not have a sink. The changing table is set up between the Infant and Infant/Toddler Classrooms but without a sink accessible to the Infant Classroom, handwashing cannot be done without leaving the classroom and using the bathroom located off the Infant/Toddler Classroom.
The Toddler/Preschool door leading to the Infant/Toddler room did not have evacuation procedures posted.
The following was observed during the visit: Two assistant teachers were alone with eight Toddler/Preschool children.

Three staff records were reviewed and found: -One staff record did not have a current PQ registration (expired 1/10/2024), and did not have evidence of a physical, completion of orientation, interview or two reference checks. -One staff record did not have bi-monthly observations from date of hire (August 2021) through June 2023. -One staff record did not have bi-monthly observations from date of hire (October 2023) through the date of this visit.

In the Infants Classroom, a child in a high chair was observed to be on the other side of the gate that separates the Infant Classroom from the Infant/Toddler Classroom with the gate closed. The Infant teacher was supervising the child but the child was technically in the other classroom, as the high chair had crossed the threshold of the classrooms. Educators must exercise appropriate supervision of the children in their care in order to ensure their health and safety at all times and must be in sufficient proximity to children at all times in order to be able to intervene quickly when necessary.

The following was found to be accessible to children: -In TODDLER/PRESCHOOL, an open box of gloves sat on top of the changing table in reach of children and bags of baby wipes were stored under the changing table and have the label "Keep out of reach of children".

Three child records were reviewed and found: -One child's record did not have a current allergy action plan (expired 10/17/2023) and the the record did not have the required asthma action plan. This record also did not have separate medication consent forms for two medications. -One child's record did not have current medication consent forms (expired 10/2023).
The current Staff Schedule was not posted in an area easily visible to educators, parents and visitors.
The attendance was reviewed in each open classroom and found: -In INFANTS, one child was not signed out and one child was not signed in.
In TODDLER/PRESCHOOL, four water bottles were not labeled with the children's names. Personal items intended for individual use by children, including but not limited to bottles, pacifiers, toothbrushes and sleeping materials, must be labeled with the name of the child for whom they are intended.
The outdoor space was observed and the following was found to be accessible to children: -A free standing basketball hoop was not secure and tippable. -A plastic toy kitchen set was not secure and tippable.
The two internal doors in the TODDLER/PRESCHOOL classroom did not have evacuation procedures posted.
One child's Albuterol inhaler was not available at the program.
One child's antihistamine was not current (expired 12/2023).
All paperwork was reviewed for children with chronic medical conditions and the following was found: -One child's record was missing an IHCP for an allergy. -One child's record was missing an IHCP for asthma. -One child's IHCP was not current (expired 12/16/2023) and one IHCP form was used for two medical conditions.
Staff:child ratios and groupings
A preschooler was observed to be assigned to the Infant/Toddler classroom for the day. Each child must be assigned to a consistent group with consistent staffing.
This investigation inspection recorded no violations or advisories.

Emergency medications readily available
Two covered wooden platforms accessible to children, had sharp corners not protected.
The door to the kitchen was ajar; the following hazards were observed: 7 gallons of bleach, a roll of plastic garbage bags, an unsecured metal paper rack. This door was locked at the licensor's request. In the hallway was a stack of eight plastic chairs. In addition to this being a tipping hazard, the hallway was obstructed in the event of an emergency evacuation. In the hallway on open shelving was a large, industrial size tape dispenser with sharp cutting edge, and a container of two dozen batteries. These hazards were removed at the licensor's request.

Sheets and blankets, in use during naptime, in the Preschool Room, were not labeled, with the name of the child for whom it is intended. Three out of eight drinking cups were not labeled, with the name of the child for whom it is intended.
There was not a current parent/guardian consent for the administration of(REDACTED) for one child.
