Three staff records were reviewed and found: one staff record is missing a bi-monthly observation (none since 11/2025) and one reference (has 1); and the second staff record is missing evidence of 1 date for MMR (has 1), 1 reference (has 1) and is missing a bi-monthly observation since 12/2025.
Five child records were reviewed and found one child’s enrollment paperwork expired 3/7/2026 and did not have a current physical.
The following was observed during the visit to be accessible to children: In PRESCHOOL, the bottom of the wall on the left side near the heater was broken; the GREEN BATHROOM had a plunger next to the toilet; the BLUE BATHROOM had a toilet brush next to the toilet; in PREK the dehumidifier was on a high shelf with the cord hanging down and it was not secure; in TODDLER B, the small circular area rug was turning up on one side (tripping hazard); in TODDLER A, the changing table pad had a small rip in the corner; and in INFANT, a large amount of sticky tape residue was on the side of the changing table.
The following was observed to be accessible to children during the visit: The fence in the Toddler area closest to the parking lot was separated from the corner post creating a gap that children could squeeze through.
During the visit, one child’s EpiPen and antihistamine had expired and another child had one EpiPen available (needs two) and was missing the antihistamine. Emergency medications must be readily available at all times.

The half doors located at the tops of the stairs leading down from the Toddler Room to the outside and from the entrance of the program down to the future preschool classrooms were not locked and could be opened by children and allow them access to the stairs.
The sink in the Infant classroom is being used for food preparation and hand washing. Licensor explained the regulation and the program came up with a plan to ensure the sinks are separate.
An entrapment hazard was found in the outdoor Toddler Area. To the left of door leading into the stairs near the Toddler Room, there is a space between the fence posts and the building approximately four inches in width. All play equipment, fences and structures must be free of entrapment hazards.
Two children's medications were stored in their respective backpacks in the hallway outside the classroom. One backpack was low enough for children to reach. These medications were not taken outside with the group. Licensor recommended the program write a policy addressing what the procedures are for handling medication that needs to go back and forth between a child's home and the program to ensure it is available at the program every day and is handed between adults only.
Two staff records were reviewed and found: -One staff record was missing EEC Essentials 2.0 (hired 5/2024) and was missing at least one observation (only observed 8/19/2024). -One staff record (hired in 7/2022) was missing all annual evaluations and has not be observed since 4/23/2024. When the Staff Records Checklist was observed briefly, licensor noted two new staff members were missing many documents needed including physicals, MMR, and EEC Essentials. Licensor explained with the exception of First Aid and CPR, all documentation must be obtained prior to staff working in a program.
Six child records were reviewed and found: -One child's record was missing a medication consent form and an IHCP. - One child's record is missing an IHCP. -One child's IHCP was incomplete. It was missing the side effects of the treatment and the order of medications to be given was incorrect (not the same as on the doctor's plan). -One child's allergy action plan from the doctor was missing.

The following was observed by Licensor in the Infant Classroom- a light in the shape of a whale was clipped onto and hanging over a crib with an infant inside. This is a violation on the Safe Sleep Policy.
The following was found to be accessible to children: -In the TODDLER ROOM, the wooden lateral bookshelf was not secure and tippable. -In the PRESCHOOL ROOM, a pair of adult scissors was sitting on the teacher's desk, the trash can was not covered, and the wooden lateral bookshelf was not secure and tippable. -In the hallway leading from the Toddler Room to the outdoor space, two large bins with outdoor play materials sat on top of a shelf and were not secure. They could fall and hurt someone.
The Building and Fire Inspection is not current (expired 8/29/2024).
A toddler climber with slide and small climbing wall (ages 2-6) was installed in the outdoor Toddler Area without prior EEC approval of the ground surface underneath the climber. Until the program provides the plans used to install the ground surface area, the toddler climber cannot be used.
The following was found to be accessible to children: - In the Toddler Area, the downspout closest to the door is rusting and chipping paint at the bottom. -In the Preschool Area, the window close to the playground has flaking wood and paint where a cord appears to have been located and the pink kitchen set is not secure and tippable.
The following was observed to be accessible to children: In the TODDLER BATHROOM, two areas on the same wall as the window have paint worn away.
The Fire Drill Log was reviewed and found documentation was missing for drills documented for January through March 2024.
Entrapment Hazards

The middle window in the PRESCHOOL classroom was open and the window did not have any protection or way to prevent a child from falling. The window was secured with window clips during this visit. All windows used for ventilation must include screens in good repair. Windows and glass doors must be constructed, adapted, or adjusted through the use of window guards or other means to prevent injury to children.
An EpiPen and an inhaler were not in their original boxes. All prescription medications must be in the containers in which they were originally dispensed and with their original labels affixed. Over-the-counter medications must be in the original manufacturer’s packaging.

Three child records were reviewed and found: -One child's record was missing an IHCP, two medication consent forms, and did not have a current physical (last physical dated (REDACTED). -One child's records has an IHCP not signed and dated by the parents.
Two staff records were reviewed and found: -One staff record was missing bi-monthly observations. This staff member began working at the program in 10/2023.
The following was observed to be accessible to children: -In TODDLERS, the toy kitchen set with not secure and tippable, a teacher's pocketbook hung on a low hook, and wipe baggies and a box of opened dryer sheets were observed under the changing table. The dryer sheets and pocketbook were moved during the visit. -In INFANTS, wipes baggies were under the changing table. -In PRESCHOOL, a pair of adult scissors were in a cup on the teacher's desk. The scissors were moved during the visit.
The following was observed to be accessible to children: -White paint along the bottom of the outside of the building of the corner in the Preschool play area was chipping.
An opening approximately 7 inches wide was observed in the white fence in the Preschool Play Area. Spaces between 3.5 and 9 inches are considered entrapment hazards.
The evacuation drill log was reviewed and found drills were not conducted in January, February, and March 2024.
The evacuation procedures in the infant classroom do not include infant-specific plans to evacuate the children.
