The following was observed during the visit: two children were missing their required second (REDACTED) and another child was missing their required (REDACTED).

Seven child records were reviewed and found: one child's record does not have a current physical and another child's doctor allergy plan is not current (REDACTED) and their medication consent form was not current (REDACTED).

Staff and children wash hands: after toileting or diapering
A review of the evacuation drill log was conducted and found there was no evidence of a drill conducted in July 2025.
The following was observed during the visit: the refrigerator in INFANT 2 did not have a thermometer.
During the visit, an educator in INFANT 1 was observed to change a child's diaper and then place the child on the floor without washing their hands. The licensee must ensure that educators and children wash their hands with liquid soap and running water, using friction, in accordance with Department of Public Health guidelines. Hands must be dried with individual or disposable towels or automatic hand blow-dryers. The use of common towels is prohibited. Educators and children must wash their hands at least at the following times: after toileting or diapering.

Two bags of food in the freezer were open and the food was exposed to the air.
Medication
One child's inhaler was not in its original box with label from the pharmacist. Another child's EpiPen and antihistamine were not in their original boxes. In ROOM 9, PREKINDERGARTEN, the backpack containing the children's emergency medications was sitting on top of the stack of cots, accessible to children (the hook where it is normally hanging was broken). The bag was relocated during this visit.

Eight child records were reviewed and found: One child record was missing a parental consent for Zyrtec; and one child record did not have a current parental consent for Zyrtec.
The following was found to be accessible to children: - In ROOM 12, PRESCHOOL, Girl's bathroom, a box of open gloves and two baggies containing wipes were sitting on top of a low shelf; In ROOM 11, PRESCHOOL, the bathroom had a plunger next to the toilet and a box of open gloves (box of gloves moved during visit to be out of the reach of children); and in ROOM 1, INFANTS, one of the sit assist pillows had two large rips.
All IHCPs were reviewed and found: Two children’s IHCPs are not complete. EEC cannot accept when parent writes “see allergy plan” or leave items blank.
One child's emergency medication was in the wrong classroom. Another child's EpiPen and inhaler were not available.

The following was found to be accessible to children: -In PRESCHOOL ROOM 11, the large brown rug under the play kitchen area was curling up at one corner and was a tripping hazard. -Two play tool benches in two preschool classrooms were not secure and tippable.
The IHCPs were reviewed for children with chronic health conditions and found: -Ten child records did not have current IHCPs.

The medication consent forms were reviewed for all children with medication and found: -Six child records did not have current medication consent forms and one consent form was used for two medications.

A review of staff records indicated that there was not documentation of bi-monthly observation for two staff.
There was not an Individual Health Care Plan for one child with a diagnosed allergy.
In EP4, plastic bags and a toilet bowl brush were accessible to children in the bathroom.
In EP4, three of seven personal drinking cups were not labeled. In PreK, three of sixteen cups were not labeled, with the name of the child for whom it is intended.

An unsecured wooden ramp (board) was accessible to children in the Toddler Room. The ramp was moved at the time of the visit.
One child was diapered; however, [REDACTED] hands were not washed immediately after. The child was observed picking up a toy before having [REDACTED] hands washed.
