The window on the loft area in the FISH CLASSROOM is an entrapment hazard. The square openings are 4 by 4 inches. Entrapment is defined as an opening between 3.5 and 9 inches.
This investigation inspection recorded no violations or advisories.

Emergency medications readily available
An open container of ice melt was sitting on the ledge of the wall to the left of the entrance to the program accessible to children. The ice melt bag next to the container stated "Keep out of reach of children".
A review of the emergency medications and paperwork were reviewed and found two IHCPs were missing.
One child's antihistamine was not available at the program.
This capacity increase visit inspection recorded no violations or advisories.

On the playground the following was observed: pieces of torn garden mulch creating trip hazards, a wire on fencing broken and protruding, a metal pipe on the ground, the round slide edge is not covered to protect from sharp edge.

A review of two staff records indicated that bi monthly staff observations are not signed by the educator, as requried.
One outdoor "sensory saucer swing" did not have the required clearance at the sides of the swing. The swing had approximately 3-4 feet from the side of the swing to the tree trunk, on both sides.
One doorway from the Turtle Room did not have emergency evacuation procedures/diagramposted.

A review of staff records indicated that not all required documentation was available, including Program Orientation, Staff Information Form, Resume and Verification of References.
One piece of equipment in the classroom had an unprotected sharp corner.
A review of Evacuation Drill Log indicated that the number of children, and the route taken, is not documented. TA was provided.
