One of the injuries a child sustained while at the program was not immediately reported to EEC. The injury occurred on 6/16/2026 and the program notified EEC via submission of a case via the LEAD portal on 6/18/2026 at 5:11pm. During this visit, Admin stated they did not know the child received stiches until 6/17/2026. Notification of Death or Serious Injury. The licensee must immediately report to the Department by telephone the following: any injury to any child which occurs during the hours while such child is in care and which requires hospitalization or emergency medical treatment. Technical Assistance provided to the program in October/November 2024 (case 00136632) after a child fractured their wrist after falling from a climber. The notifications to EEC were reviewed with Director Ashley at the time as the program did not immediately notify EEC by phone of the injury resulting in emergency medical treatment. That injury occurred on 10/24/2024, the program staff noticed the child favoring their opposite arm on 10/25/2024 and communicated their concerns with the child’s parents, the diagnosis was made on 10/28/2024, the child's parent signed the injury report on 10/29/2024 when the child returned to the program, and the program entered the case in LEAD to notify EEC on 10/30/2024.
