The following was observed to be accessible to children during the visit: outside in the preschool area, the space between the two gates was approximately 4 inches wide and a child could potentially slip their body through the space to leave the licensed outdoor area.
The following was observed to be accessible to children during the visit: the ceiling vents throughout the program were dusty, the toy sinks in PRESCHOOL B were dusty and hair was visible to be in corners, a plunger sat next to the toilet in the TODDLER B bathroom and a floor tile was cracked, tape residue was visible on the outsides of the bookshelves in TODDLER D, both bathrooms in PRESCHOOL A had toilet brushes and plungers, the metal sewer cover outside of the PRESCHOOL B classroom door in the hallway no longer stays in place as the screws were missing, and the hallway leading from the entrance to the classrooms had many floor tiles bubbling and cracking and brown stairs were visible outside of each classroom.

The following was observed during the visit to be accessible to children: In INFANT B, there are multiple spots of peeled/chipped paint near the window; the inside of the door in TODDLER B had a large area of peeling paint; and in PRESCHOOL B, paint was chipping/peeling on the wall below the Alphabet Chart and there was a hole in the wall near the outside exit door with chipping paint and plaster showing.

Three staff records were reviewed and found one was missing current First Aid certification (expired 4/27/2026).
During the visit, the staff meetings were reviewed and found monthly staff meetings were not held in June or December 2025.
On 5/13/2026, staff names on the Staff Schedule were cross referenced with staff names in the BRC Navigator and found two current staff do not have current BRCs.
The following was observed during the visit: children's soft rest materials in Preschool B were sitting on top of the cubbies and not separate. They were all touching. The licensee must provide sufficient space, accessible to children, for each child to store clothing and other personal items in a safe, sanitary manner.

In the PRESCHOOL B bathroom, the diapering and toileting procedures were missing and in the PRESCHOOL A bathroom, the toileting procedures were missing. A written plan for diapering and toilet training and for the disposal or cleaning of soiled clothing, linen, blankets and diapers must be developed, implemented and posted in diapering areas.
During a review of children's emergency medications, it was found that an (REDACTED) was missing for one child (REDACTE).
The refrigerator in Preschool B did not have a thermometer. Thermometers verifying temperatures must be maintained in all refrigerators.
The following was observed during the visit: In PRESCHOOL B, when a child sustained a cut lip after falling and one teacher attended to them in the bathroom to clean up, a pair of adult scissors was left on the table where children were sitting and a child picked up the scissors. Another child picked up a staple from the floor and brought it over to the licensor to show him/her.
The Building and Fire Inspection is not current (expired 12/26/2025). The IPMs are current and expire 5/4/2027.

The following was observed to be accessible to children: In the Toddler area, a free standing basketball hop was not secure and tippable.

The following was observed to be accessible to children: In TODDLER B, paint is peeling in a couple of spots on the outer wall near the bookshelf and on the low window on the inside of the room; and in TODDLER D, paint is peeling under the "Spring" bulletin board and two holes with visible plaster are on the wall behind the play stove and sink.

Three staff records were reviewed and found one staff record is missing a physical and evidence of immunity to MMR.
The following was observed: In TODDLER C, ten children were signed into the room but only nine were present during the visit. The attendance was corrected during this visit to reflect one child transitioned to Toddler D.
The following was observed to be accessible to children: In INFANT A, two ceiling tiles above the classroom sink were stained; In TODDLER B, dust collected on the surface of the play sink behind the bucket, under the changing pad, and a toilet brush was found in the bathroom next to a toilet; In TODDLER D, the carpet had two pulls with short stings, the empty sensory table was collecting dust and crumbs, and the area rug on the block area did not fit the space and one end was creating the corner to curl up (tripping hazard); In TODDLER C, a floor tile between the gate separating TODDLER C from TODDLER D is missing (tripping hazard), and sticky tape residue was found on the half wall near the insect pictures; and ceiling vents throughout the program were caked in dust and the floors throughout the program appeared discolored, dusty and dirty.
The program stated they are waiting for their current Health Inspection to be sent to them.
Paint was observed to be peeling in the following areas: -In PRESCHOOL B, in large areas below the two bulletin boards. -In TODDLER B, along the half walls separating the classroom from the neighboring space.
One child's IHCP was not complete. It was missing the potential consequences.

Upon reviewing staff records, it was observed that one staff member took an online course for First Aid and CPR. This does not fulfill the regulation and cannot be accepted by EEC.
Seven child records were reviewed and found: -One child's record was missing the evidence to place the child in an (REDACTED). Director reported this was at the request of the parents upon enrollment and (REDACTED).
Three staff records were reviewed and found: -One staff record was missing evidence of MMR immunity. -One staff record was missing bi-monthly observations from the past year (hired 6/2022).
The following was found to be accessible to children: -In INFANT A, the changing pad had a rip as well as the sitting support pillows. -In TODDLER B, the changing table lock was no longer effective and allows child access to the ointments inside (have labels to Keep Out of Reach of Children); the play stove and sink were dirty (dust and crumbs visible); and tape residue is on the back of the aqua and white bookshelves. -In TODDLER D, two of the low aqua and white bookshelves had holes beginning on the end caps and the composite material is exposed; four ceiling tiles are stained near the door; and the ceiling vent is dusty. -In TODDLER C, the toy sink is dirty (crumbs and dust) and tape residue is visible on the tall white cabinet to the right of the sink and on the half wall separating the classroom from the neighboring space.

A review of five child records was conducted and found: -One child record was missing a physical description or picture.
The staff schedule was not posted in a visible location for parents and visitors.
The following was observed to be accessible to children: -In PRESCHOOL B, a stapler was in the right hand drawer under the sink. -In the TODDLER D BATHROOM, a toilet brush sat to the right of the toilet and plastic bags of wipes were sitting on low shelves.
The following was observed to be accessible to children: -In the TODDLER PLAYGROUND, the wooden shed had a nail sticking up approximately half an inch and the tree roots are emerging from the ground and are considered a tripping hazard.
The following was observed: -In the TODDLER D bathroom, paint brushes were laying on the back of the sink indicating they had been washed in the bathroom sink where hands are washed.

Emergency medications readily available
A toilet brush was standing in a holder on the floor in the bathroom of TODDLER B and accessible to children. A toilet brush contains germs and should be kept out of reach of children.
Not all furniture in the program was anchored. Unanchored furniture is considered tippable and a hazard to children if the furniture should tip over. The following furniture observed in the program that needs to be anchored: low wooden shelf in INFANT B; wooden bookshelf in TODDLER B; and wooden bookshelf and wooden sink/hutch in PRESCHOOL A.
The ground surface material (mulch) was approximately an inch in depth at the bottom of the slides on the toddler and preschool climbing structures. The ground surface material must meet a certain depth to prevent serious injuries. The depth for mulch is required to be at least nine inches.
Postings that state where the First Aid Kit were missing in the following rooms: INFANT B and PreK B.
All staff were aware of the health needs of the children in their classrooms. One IHCP for a child with asthma and an inhaler was missing the names of staff trained to administer.
Two(REDACTED) in room PRESCHOOL A were not kept in the classroom. They were kept in the Director's office in a wall cabinet. Emergency medications such as (REDACTED) auto-injectors must be immediately available for use as needed.
Equipment is clean, safe, secured, and poses no hazard.
