The following was observed to be accessible to children during the visit: In PREK 2, the cords under the computer table were loose and in a pile. Cords must be contained and in order so children cannot wrap them around appendages.

The Evacuation Drill Log was reviewed and found the time elapsed was not documented during drills in August, September October or November 2024 and the doors used were not consistently documented. The educator must hold practice evacuation drills with all groups of children and all educators from each floor level of the approved space at least monthly. Drills must be held during different times of the program day, and must use alternative exits. The educator must document the date, time, exit route used, number of children evacuated and effectiveness of each drill.
Three staff records were reviewed and found: -One staff record does not have evidence of First Aid certification, is missing an annual PQ Registration, and does not have an annual evaluation nor have they been observed since they were hired in 11/2023. -One staff record does not have a current PQ registration (expired 2/22/2024), and is missing bi-monthly observations from before 9/17/2024 hired in 2014). -One staff record does not have a current physical (last one 1/12/2022), does not have a current PQ registration (expired 1/9/2024) and is missing bi-monthly observations between 7/2020 and 9/23/2024.

The following was observed to be accessible to children: In PREK 1, a staple remover was inside an unlocked drawer; in PREK 1B the top left drawer is broken and unlocked (could pinch fingers when opened); in TRANSITIONAL KINDERGARTEN the kitchen stove was not secure and tippable, the long white computer cord was not secure to the wall, and two rolls of loose bags were found in the unlocked cabinet under the sink; and the bathroom on the right side in the hallway had an unlocked metal cabinet with six bottles of skin cleaner that state "Keep out of reach of children" on the label.
The following was observed to be accessible to children: In the outdoor Toddler area, an umbrella with umbrella stand is not secure and tippable.
IHCPs were reviewed for all children: -One child does not have an IHCP for a chronic condition. -One child’s IHCP states the names of the staff who are trained to administer the medication is “teachers”. -One child’s IHCP is incomplete and is missing all of the details. -One child’s IHCP is incomplete and is missing side effects and names of staff who can administer medication.

Attendance Recording and Group Sizes/Ratios
Observations from original visit on 2023-11-28 00:00:00: The following was observed: -The two separate PREK 1 classrooms were using one attendance sheet and it was hanging in the PREK 1B classroom, leaving the PREK 1 classroom without their own attendance. -PREK 1B was operating out of ratio. One educator was assigned to eleven children. The Assistant Director working in this classroom for the day stated she thought it was acceptable for an assistant teacher to float between the two PREK 1 classrooms because they were in ratio as they were treating the two classrooms as one group. Corrective Plan received on 12/1/2023: The PREK 1 classroom has been separated into two separate classes and a separate roster has been created for PREK 1 and PREK 1B classes. One group now has 10 children and the other group has 10-15 children correcting the ratios. Support teacher is assigned to larger group. Visit 1/5/2024: The following was observed: -The PREK 1 and 1B classrooms now have separate attendance and were in ratio. -The PREK classroom had inaccurate attendance. Ten children were present and nine were marked as present. The attendance was updated during the visit.
Observations from original visit on 2023-11-28 00:00:00: The following was observed to be accessible to children: -In the LEFT HAND SINGLE KINDERGARTEN BATHROOM, the unlocked metal cabinet was rusting along the bottom and contained four cans of "Phenol-Quat" cans that state on the label "Keep out of reach of children". -In the RIGHT HAND SINGLE KINDERGARTEN BATHROOM, the unlocked metal cabinet was rusting along the bottom and contained two bottles of foaming soap and four bottle of "Apothecary Soap" all with labels that state on the label "Keep out of reach of children". Corrective Plan received on 12/1/2023: The cans of Phenol-Quat were removed immediately after the visit. All the foaming soap has been placed in the high wall cabinet out of reach by the children. Shelving mats have been purchased to cover the rusted shelving. The exterior rust will be scraped and repainted. This is a projected schedule during our winter break 12/25 - 12/30. Visit 1/5/2024: The bathroom cabinets were observed and although all of the cleaning products have been removed, the cabinets are still rusty along the bottom.

Observations from original visit on 2023-11-28 00:00:00: The following was found to be accessible to children: -In PREK 1, the 4-cube shelf to the left of the sink and the metal bookshelf with phone on top were not secure and tippable and the phone jack was not covered. -In PREK 1B, the two small white cube shelves, the toy kitchen set, and the metal bookshelf were not secure and tippable, and the phone jack was not covered. -In the PREK 1B BATHROOM, a loose trash bag was tucked in the corner under the sink. The bag is a suffocation hazard. The bag was removed during the visit. -In the TODDLER/PRESCHOOL BATHROOM, eight baby wipe bags were in open bins on open shelving and are suffocated hazards. -In KINDERGARTEN, three metal bookshelves have sharp corners accessible. -In PREK 2, three 4-cube shelves, a Barbie house, and toy kitchen set were tippable and not secure and the metal bookshelf has sharp corners. Corrective Plan received on 12/1/2023: All shelving has been reviewed and will be secured together or to the walls. Cushion tabs are being applied to all metal shelving to prevent corner exposure. New pocket hangers have been purchased to place on doors for wipes which will be out of reach to children. Kitchen and toy Barbie house will get mounted to the walls. Phone was removed from shelf and is now on counter out of reach to the children. Some work had to be schedule for these adjustments and will be done during our winter break 12/25/ - 12/30 when children are not in the facility. Visit 1/5/2024: The following was observed to be accessible to children: -In PREK 1, the 4-cube shelf to the left of the sink is tippable and not secure and the phone jack was not covered. -In PREK 1B, the phone jack was not covered. -In PREK 2, one 4-cube shelf next to the computer desk and the Barbie house were tippable and not secure.
Observations from original visit on 2023-11-28 00:00:00: The following was observed to be accessible to children: -In TODDLER/PRESCHOOL on the blue painted wall two spots were chipping and the plaster underneath was visible. Corrective Plan received on 12/1/2023: This area has been covered with tape but will be replastered during the winter break 12/26 - 12/30 when other repairs or adjustments can be made without the children present. Visit 1/5/2024: The two spots on the blue wall in the Toddler/Preschool classroom were still observed.

During the visit, Licensor observed the educator working in PREK 1B leave the classroom to get sleep cots from the hallway twice, leaving the children in the classroom without supervision.
The following was found to be accessible to children: -In the TODDLER outdoor space, a free standing umbrella was sitting in a base but it was tippable. -In the PRESCHOOL outdoor space, an area of tree roots is exposed from the ground and is a tripping hazard (located at the edge of the rubber mulch and square floor tiles).
All emergency medications for children and the corresponding medical paperwork was reviewed and found: -One child's IHCP was not complete and stated "See attached". -One child's IHCP was not complete. It was missing the name of the educators trained to administer the medication and who trained them. -One child's IHCP was not complete. It was missing the medical condition, consequences of not receiving the treatment, and names of educators trained to administer medication. -One child's IHCP was not complete. It was missing the condition. -One child's IHCP was not current. It (REDACTED).

During a review of three staff records, it was found one staff member did not have current First Aid and CPR certification. It expired 6/30/2023.
Five child records reviewed and the following was found: -One child's record was missing a consent for their antihistamine. -One child's record did not have separate consent forms for their EpiPen and antihistamine. -One child's record was missing their seizure plan, did not have a current enrollment form (last dated 9/13/2021) and did not have a current transportation plan (last dated 9/13/2021).
The following was observed: -The two separate PREK 1 classrooms were using one attendance sheet and it was hanging in the PREK 1B classroom, leaving the PREK 1 classroom without their own attendance. -PREK 1B was operating out of ratio. The SCHOOL AGE classroom did not have evacuation procedures posted at the exit to the lobby nor at the exit to the PREKINDERGARTEN 2 classroom.
A review of the emergency evacuation log was conducted and found a drill was not documented in February 2023.

All classrooms with the exception of Kindergarten stated they do not take their emergency medications outside when the group transitions outside. Emergency or life-saving medications, such as asthma inhalers and epinephrine auto-injectors, for any children for whom they have been prescribed.

The following hazards were observed in the outdoor play space: Infant Toddler Playground: fencing screws protruding, standing water, insufficient fence height to prevent access to side of building. Preschool Playground: leaning against the building a ladder, black metal gate parts, broken equipment, netting, garden hose draped as a trip hazard, insufficient depth of impact absorbing material under and around slide, (which is approximately 5 feet high), an unassembled playhouse, plastic kitchen set not secured, plastic tarp from sandbox laying on ground, and accessible to children.
One (REDACTED) was not in the box, with label, to identify to whom it is prescribed.
In the toddler room, staff indicated that seven children were present. They then counted the children and said 6 were present, which was also what the attendance record indicated.

Two educators was not aware that (REDACTED) was prescribed for a child with a documented (REDACTED). One child's Individual Health care Plan (IHCP), for an (REDACTED) did not include all required information; additionally this document was not signed and dated by parent. One child's file did not have documentation of an IHCP for (REDACTED).
