The plan for diapering was not posted in the Infant Room and a changing table is in this room. A written plan for diapering and toilet training and for the disposal or cleaning of soiled clothing, linen, blankets and diapers must be developed, implemented and posted in diapering areas.

Plan for diapering and toilet training
Three children's' records were reviewed during the visit and found two children's records do not have current enrollment forms, developmental histories, CPR/first aid consent forms (expired 9/4/205 and one not signed/dated), or annual physicals (expired 8/30/2025 and 7/19/2025).
Two staff records were reviewed during the visit and found: One staff record was missing a bi-monthly observation.
The program does not have a medication consent form filled out and signed by the parent allowing them permission to administer the medication to the child.

Three children’s records were reviewed and found: One child record was missing a transportation plan; and one child record was missing a medication consent form signed by the parents, and the allergy plan written by the doctor.

Individual Health Care Plan (IHCP) - R Apr 2025
Two staff records were reviewed and the following was found: One staff record was missing evidence of completed EEC Essentials 2.0, and annual PQ registration. One record was missing their current First Aid and CPR certification, bi-monthly observations and their orientation checklist was incomplete (missing the signature of the administrator who conducted the orientation and the number of hours).
No evidence could be found for staff meetings held September 2024 through January 2025, nor March 2025.
One IHCP was reviewed and found it was incomplete. The names of the educators trained to administer the medication were missing.
Two child records were reviewed and found that one child record was not complete. The medication consent for emergency medications and the authorization to allow staff to provide basic first aid and CPR were not signed and dated by the child's parent.

One child's antihistamine was not in the original box.
The Evacuation Drill Log was reviewed and found drills have not been conducted since the program opened.

The following was found to be accessible to children: In the bathroom, two open boxes of gloves were sitting on the low window sill next to the toilet and a loose screw was in the top draw under the sink. All of these items were moved during the visit.

One staff record was reviewed and found the following is missing: evidence of completion of EEC Essentials 2.0 for GSA, a physical, and evidence of MMR vaccination (2 dates or a titer test).

Evacuation procedures posted at each exit
The Fire Drill Log for 2022 was reviewed; this document indicated that no evacuation practice was done in January, February, March, April, July, and August of 2022.
Four children were listed as present on the attendance sheet; however, thirteen children were present at the time of the visit. In addition, one group on the playground did not have an attendance with them.

Food prepared/served in safe/sanitary manner
The Toddler Preschool Group totaled 10 children on the day of the visit. The classroom educator stated that it is sometimes 10 children, because some children are part time. EEC regulations state that the maximum number of children in a mixed Toddler/Preschool group is 9 children.
Three Infant/Toddlers were observed being seated into their high chairs, served their food and eating their food. Their hands were not washed prior to lunch. The program's refrigerator was "out of order"; two infant bottles were stored in lunch bags, with an ice pack. There was no method to ensure that the milk was at a safe temperature.
Emergency evacuation procedures were not posted at the back door; when discussed, the Director found them in the kitchen and taped them to the wall.
The following hazards were observed, and accessible to children: The gate to the kitchen is not working and not secured to prevent access to the space; in the kitchen a teachers purse and container of bleach; plastic bags in bathroom. The hazards were moved during the visit.
Nine children were present at the program; however, only five were signed in on the attendance sheet.
Evacuation drill log was not available. During a follow up phone call the following day, the Director stated that no emergency evacuations were practiced since September 2021.

Upon arrival at the program, one child was observed in a pack and play, standing up. No staff person was in the room at the time. Less than a minute later a staff person entered the room.

Building and Fire Inspection Certificate was not current; expired 10/25/2021. The Director stated that the building was inspected this year, and will follow up on the Certificate.
