During, the visit Licensor reviewed four random records of the children currently enrolled. The following was observed: 1) a child was missing emergency card information in the enrollment packet. 2) a child was missing acknowledgement of receipt of parent handbook signature and date in the enrollment packet.

1) Child {REDACTED} needs an individual health care plan. According to his medical, he has {REDACTED}. Developmental Profile was blank. 2) Child {REDACTED} needs {REDACTED}. 3) Child {REDACTED}: needs {REDACTED}. The educator has two children with {REDACTED}. She was told that these children need progress report every three months.
The educator did not have a written plan for sheltering in place and lockdown procedure.
The educator has a plastic diaper changing surface. She did not have a covering of adequate size to prevent the from coming into contact with the changing surface.
The refrigerator’s temperature was 40F. The educator did not have a thermometer for the freezer.
The educator has 9 children enrolled in her program. Licensor observed 6 toothbrushes. The educator needs two toothbrushes. She reported taking care of an {REDACTED} that doesn’t have any teeth. She was told to use a swab to clean the infant’s gums after drinking a bottle.
The water was too hot to the touch. Discussed with the educator that the water temperature must not exceed 120F.
Licensor observed a broken glass window in the playroom area.
The educator has a child with a medical condition. She needs an emergency medical information list posted in a confidential manner.
The transportation plan was incomplete; missing the following information: 1) how transportation is being provided; vehicle model; number of passengers seats and available car seats 2) how the driver will respond to disruptive behavior, including children unbuckling seat belts or refusing to remain seated. 3) information about first aid kit, seat belt cutter, window smasher, and emergency numbers available in the vehicle.
