A staff records review of six records performed during the visit reflected the following: a) One record did not contain evidence of a current health physical and MMR immunity. b) One record did not contain evidence of a current first aid training. c) Five records did not contain evidence of ongoing bi-monthly observations. d) Three records did not contain evidence of annual evaluations.

At the time of the visit educators were not exercising appropriate supervision of the children while children were using the restroom.
At the time of the visit EEC reviewed four staff records and it reflected the following: a) Two records contained insufficient evidence of teacher qualifications. b) Three records contained no evidence of orientation forms. c) Three records contained no evidence of current health physicals and MMR immunity. d) Four records contained no evidence of EEC essential trainings, ongoing bi-monthly observations nor annual evaluations. e) One record contained no evidence of staff info form, two reference checks nor interview notes.
At the time of the visit both the building inspection and the fire inspection were expired.

At the time of the visit three staff members did not have a completed background check.
At the time of the visit one of the classrooms was not able to provide EEC with an accurate attendance. And the attendance provided by the Program Director did not reflect all of children in attendance.
At the time of the visit one staff member did not wash their hands or wear gloves while serving a child.
At the time of the visit the heating source was not covered in a few different areas of the classrooms causing a potential hazard for children.
At the time of the visit disinfecting wipes were stored in an unlocked cabinet in the children's restroom and therefore accessible to children.
At the time of the visit EEC observed the following: a) Multiple children did not wash their hands after toileting. b) One staff member did not wash their hands after changing diapers and supporting children with toileting.
At the time of the visit EEC observed the following: a) Two incomplete IHCPs. Both IHCPs were missing required information as well as the physician's signature and/or parent's signature. b) One medication listed on an IHCP not available for review.
