Space (indoor / outdoor hazards)
The licensor observed that the educator had certified assistance attendance sheet with a time in and not a time out. The certified was not present at the time of the visit.
The licensor reviewed three different children records and observed the following: 1) Child #1 has an expired physical, immunization and lead test. 2) Child #3 does not have an updated lead screening test in child file.
The licensor observed that the educator had handed out a snack to a child without properly washing his hands.
The licensor observed that the second exit was covered with snow and was not accessible at the time of the visit.
The licensor observed the following indoor hazards: 1)The water temperature was 130*F at the time of the visit. 2)The smoke alarm in the daycare room was leaning on the side of the wall and removed from its original ceiling position.
Although fire drills are documented to be happening monthly, Educator is only conducting AM drills. Last fire drill documented is from 2.3.25
Licensor observed the following indoor hazards: Bathroom: 1. Water temperature was 140*F, exceeding the limit of 120*F. 2. Unlocked under the sink drawer containing beauty products. Kitchen 1. Uncovered outlets within the reach of children.

Although Licensor observed a aren’t handbook it was missing information in emergency section and did not explain guidance behavior methods conducted by Educator.
Although Licensor observed an emergency preparedness plan it was missing the following information: location for monthly fire drills, location for evacuation, lockdown procedure and what educator is going to do with children in the program if she has to look for missing child.
Missing updated Medical Forms for some of the children enrolled in the program.
