Individual Health Care Plan (IHCP) - R Apr 2025
The heating vent in the yellow preschool room is rusty.
A review of five IHCPs showed that one did not list an [REDACTED] as part of the plan, however, the physician's allergy action plan listed [REDACTED] as part of the plan. The parent supplied cetirizine as the antihistamine.
A student's [REDACTED] was expired.
A review of three staff files showed two staff members did not have completed BRCs.
A review of emergency medications showed an expired [REDACTED]
A container of Lysol disinfectant wipes was observed stored within reach of children.
A review of the staff records checklist and three staff files showed the following: * One staff file was missing evidence of orientation, reference checks, documented interview, * One staff file was missing physical, proof of [REDACTED]orientation, reference check, and documentation of interview
A review of the children's record checklist showed several students were missing physical and immunization records.
The heating cover in one bathroom was covered in rust.
A review of one IHCP showed it to be expired and missing the required side effects of treatment and training information.
The dramatic play furniture in the Five-year-old classroom was not secured.

A review of the attendance in the yellow preschool room showed 13 children signed in while 14 children were present.
An educator was observed in the Five-year-old classroom using bleach to sanitize a table without first washing the table with soap and water.

At the time of the visit, the following outdoor hazards were observed: 1) A little tyke basketball easy score basketball set was not secured, and the set was seen swaying and almost falling over when children bounced the basketball off the backboard of this piece of equipment. 2) A little tyke slide was positioned within the fall zone of the larger installed playground structure.
The program's emergency and evacuation procedures were not posted at the exits of Williams Hall.

Evacuation procedures posted at each exit
The program did not have a current medication consent in Child A's file for an emergency medication that was prescribed for this Child and part of the child's Individual Health Care plan.
Of the three files reviewed, one of the files reviewed was for a staff member who the Director reported had been hired a month earlier. Although the two other staff files had all the required regulated information the newly hired staff file did not have evidence of the following: 1) Staff form 2) Verbal verification of references 3) A health physical within the year of date of hire 4) Evidence of immunity to MMR 5) Orientation to the Program
Through a review of the yellow preschool classroom and Williams Hall (approved indoor space) the following was noted: 1) There were three stand up fans and one large flag stand in Williams Hall accessible to children. 2) Unprotected outlets, and one outlet plate was missing, in Williams Hall. 3) The door to the kitchen, and the door of Williams hall (that acts as the barrier to the top of the stairs) were not closed when children were observed using this hall for music class. 4) Plastic wrap and baggies were found in a drawer in the hallway that leads to the classrooms.
Although the program had a posting that indicated chronic health conditions, medications, food sensitivities, and food preferences, it was not current and up to date for all of the enrolled children's needs/information in the yellow classroom or on the program's refrigerator.
