Licensor randomly reviewed three different childrens files. One file had a expired medical paperwork and another file had no medical paperwork. Please acquire updated physicals and lead testing (when applicable) for all children and make a plan to renew annually.

Provider stated they have completed trainings but they were not available for review during visit as certificates were not printed out. Please make sure to have 10 hours of training per year and available for review upon visit

Wooden toy shelf with bins was not secured to the wall in the daycare room and poses a tipping hazard.
Napping Materials
Above ground pool is surrounded by a metal fence; however, the licensor observed the gate to the pool area was open and not locked as required. A large metal seesaw in the play space did not have the required fall zone material and a small trampoline was observed on the deck. Trampoline use is prohibited according to the regulations. The educator stated that this equipment is only used by her own two children, however, it is accessible to the daycare children and must be removed or made inaccessible to them by a separate fence.

The educator is not completing written progress reports as required by EEC regulations. Written progress reports must be completed every three months for infants and children with special needs, every six months for toddlers and preschoolers, and once a year for school aged children (5 years and older).

Wooden toy shelf with bins was not secured to the wall. Also, 5 wooden shelves throughout the daycare room were not secured and pose a tipping hazard. This equipment must be attached to the wall or the floor.
Cots and a crib are used for nap time. Blankets were observed to be stored touching in a pile in the crib. Discussed napping items being stored in labeled extra-large Ziploc bags or individual bins.
