Licensor reviewed two children's files and found the following: Both files were lacking a current physical on file and enrollment packets have missing signatures for permission to transport to medical facility in case of emergency. Please acquire physicals and signatures and make a plan to ensure all paperwork is renewed annually.

Provider had no trainings for licensor to review during visit. Please start taking 10 hours of training per year with 1/3 in diverse areas.
Licensor did a walkthrough of all indoor/outdoor licensed space and found the following hazards: Front door entrance has chipping paint. Please remove all chipping paint from front deck and railings.
Provider needs to add emergency procedures to Parent Handbook specifically including locations where program will meet up in case of an emergency in the house and emergency on the street/local area. Please upload Parent Handbook to document library once complete with newly added info.
Provider has a deck that will require a child proof gate at the bottom to deter children from accessing stairway while outside.
Provider needs to build a fenced in outdoor play area with at least 75 sq ft per child.
