The program director was observed alone with 16 students. It was learned that a staff member was absent without notice. During the visit and additional staff member arrived putting the program back into compliance.
Through interviews with two educators it was noted that while they knew there was a student who had an allergy the required an [REDACTED] they were unaware of what the allergy was.

During the visit an employee was observed leaving the classroom causing one educator to be alone with 14 children. On another occasion, an employee was observed alone with 15 children.
A review of the children's record checklist and 8 children's files showed they were all missing the transportation form. * 4 of the files reviewed were missing complete emergency first aid and consent forms. * 4 of the files reviewed had expired emergency consent forms *One students paperwork indicated there was either a custody agreement or restraining order on file, however the file was missing a copy of such

Emergency medications readily available
Through a review of emergency medications it was noted that one medication [REDACTED] had expired on [REDACTED] and one medication [REDACTED] was not stored in it's original box, nor had a [REDACTED] attached to it. The allergy list also indicated that some children who attended the program had [REDACTED] and [REDACTED] at the program, however there were no [REDACTED] found within the "indoor" emergency medication bag. The [REDACTED] reported that the program has two sets of emergency medications for every child with such prescribed, and one set is stored in the "indoor emergency bag" while the other set is stored in the "outdoor emergency bag".
The program did not have a method of knowing where any child is at any given time after they arrive to the program as on the day of the visit the following was observed: 1)Children were seen moving from one area of the licensed classrooms/space on the basement floor to another area without any communication between staff 2) The staff person responsible for meeting the bus (6 runs)was observed taking the master attendance documentation with [REDACTED] every time [REDACTED] went outdoor to meet the bus leaving no record of who was already signed into the program The Program was also not recording the arrival and departure times on the day of the visit.
At the time of the visit, the following was noted: 1) Children who got off the bus, entered the program without staff aware of them entering, as there was no communication between the staff outdoor meeting the bus and the program staff indoors. Children were allowed to move from one area of the basement licensed space to another area of the basement licensed space without staff knowing that children were changing classrooms.
At the time of the visit, there were no staff members present who had current First Aid and CPR certification.

A review of three staff files showed the following: *One staff file was missing the required documentation of orientation and school age work experience. *One staff file showed their last physical was completed in 2016 * One staff file was missing the required documentation of orientation *Two staff files were missing the required two reference checks
Two tall shelves in the upstairs art room and two tall black shelves in the downstairs art room were unsecured.
An educator without a current first aid certification was observed providing first aid to a student.
A review of three staff files showed two of the staff members did not have current first aid and CPR.
The corners of a rug in the "Big Room" were curled up, causing a tripping hazard.
A review of a student's IHCP showed an expired medication consent for their emergency medication.
One student's [REDACTED]expired in March 2022.
One student's emergency medication was shown to have expired in March of 2022.
A review of student's IHCPs showed one to have expired in October and another student who had food allergies noted in their file did not have an IHCP on file.

At the time of the visit, not all children washed their hands prior to eating snack.
