Through staff interviews it was determined the program director was not adhering to the designation of administration multiple site plan of visiting the programs once a week.
Through a review of the site coordinator's file, due to lack of documentation, it could not be determined that she was qualified as a site coordinator.
Individual Health Care Plan (IHCP) - R Apr 2025
A review of 4 students' IHCP's showed there students were missing complete IHCPs. One IHCP was missing the training piece, 2 students did not have parental permission to carry their emergency medication. Three students were missing the medication consent form.
