A review of one staff files showed it to have an incomplete staff form and was missing the required proof of orientation.
A student was carrying their own emergency medications without written consent from the parent. The student's IHCP indicated the need for [REDACTED]however there was none provided to the program.
This monitoring visit inspection recorded no violations or advisories.
Designation of Administration/Multiple Site Plan
Through interviews with the program staff it was determined the program administrator is not following the designation of administration/multiple site plan of being on site once a week.
Upon review of IHCP and medication, it was observed a child was carrying [REDACTED] without written consent from parent.
