An educator was observed standing a toddler up on the changing table to snap their "onesie", not having a clear grip on the child should they fall.
The following was observed in teh Great Horned Owls Preschool classroom: 1. Several tiles around the toilet were missing. 2.The dramatic play area and several shelves were very dirty

This change of space visit inspection recorded no violations or advisories.
A costume/mirror shelf was not secured in the Ducklings classroom. A kitchen playset was not secured in the Robin room.
Large, dried beans were observed in the Ducklings sensory table which present a choking hazard.
A review of medication consent forms showed one was missing the required parent signature.
The large round rug in the Ducklings classroom was very dirty.

Dirty and old playground equipment was observed stacked up in the corner of the infant playground creating a hazard.
A review of four children with IHCP’s showed them to be incomplete and missing required information: who trained and who was trained, consequences if treatment is not administered, possible side effects of treatment.

A review of the inspections showed the fire inspection has expired.
There was 2 containers of expired milk in the refrigerator and one package of expired crackers in the snack storage.

This monitoring visit inspection recorded no violations or advisories.
Plan for diapering and toilet training
At the time of the visit the following indoor items posed a potential hazard to children: 1) Plungers accessilbe to children in the bathrooms 2) A cord to a "light table" draping in the Wild Turkey and the Heron classrooms 3) A sharp metal clip sticking out from a bookshelf in the Wild Turkey classroom 4) Disposable gloves were found accessible to children in the bathroom closes to the Hummingbird classroom.

The Chickadees classroom did not have sufficient art and dramatic play materials.
The corner of the rug under the Lego table in the school-age "den" is rolling up, causing a tripping hazard.
One student’s [REDACTED] was stored in their backpack within reach of children.
The following was observed while doing a health and safety inspection in the classrooms. * A plunger was observed in the toddler bathroom. * A container of sanitizing wipes was observed stored under the sink in an unlocked cabinet in the Wild Turkeys classroom. * A can of bug spray was stored in a n unlocked drawer in the Heron's classroom * Diaper cream was observed stored in an unlocked cabinet within reach of children in the infant classroom.
There was not an adequate depth of mulch around the climbing structure.
Through a review of eight staff files it was noted that one file did not have evidence of immunity to MMR, and two files did not have evidence of current health physicals.
Through a review of children's files it was noted that four files did not have evidence that progress reports were being completed with the frequency that is necessary based on the child's age or identity of having a special need.
At the time of the visit, multiple school age children were observed going to the basement area classrooms unsupervised, without any staff positioned on that floor.

Through a review of eight children's records the following was noted: 1) One child's file did not have evidence of a current face sheet, 1st aid/emergency medical consent release, and transportation plan. 2) One child's file did not have evidence of lead screening.
At the time of the vsiit, the following equipment posed a potential hazard to children as the equipment was not secured: 1) Wooden Kitchen play kitchen set and toy shelf in the Great Horned Owls 2) Plastic toy kitchen set in the Hummingbird classroom
At the time of the visit, the Pre-K 1 bathroom did not have a diapering/toileting plan posted.
Through a random review of emergency medication consents the following was noted: 1) One child who has a inhaler at the program to use as an emergency medication, did not have a current parent consent on file for the program to administer if need be. 2) Medication consents for the program to administer an epi-pen and Benadryl as emergency medications to a child if need be were not signed by the parents
At the time of the visit, the Educators in the Hummingbird and Chickadees's classrooms did not clean and sanitize the eating surfaces prior to the children eating lunch.
Through a review of individual health care plans (IHCP) the following was noted: 1) One child's individual health care plan did not have information about whom the doctor authorized to train in the program and what staff were trained in this child's health needs. 3) One child's IHCP did not have the parent's signature, information about the side effects of the prescribed emergency medication, what happens it he medication is not given, whom the child's physician authorized to train staff at the program and which staff are trained 2) An Educator who floats to cover breaks in multiple classrooms was not knowledgeable about the IHCP's in the pre-k 1 classrooms
The Licensing Specialist observed multiple Educator helping children blow their nose. One on occasion the Educator and child did not was their hands, and on a second occasion, the Educator washed her hands only, but did not instruct the child whose nose was blown to wash his hands.
