Observed a 4-month-old asleep and strapped in a car seat with a blanket inside a pack 'n play. Any infant that is asleep in a car seat, bouncy seat or other equipment must be moved to a pack 'n play immediately. Infants under the age of one year must be placed on their backs on a flat surface when sleeping with no blankets or toys. Licensor must conduct a follow-up Safe Sleep visit in the next ten days. The parent of the child must sign the Safe Sleep Parent Letter, and this form must be kept in the child's file. The licensor will review this signed form during the follow up visit.

EEC evacuation log was reviewed and last drill logged was in August 2024. Evacuation drills must be conducted with the children and recorded every month.
First Aid Kit (FCC)
Educator'S [REDACTED]Regular Assistant [REDACTED]License expires 12/7/23. The educator did not have a file for [REDACTED]with his current medical, CPR/First aid card, and professional development training certificates. Sanjay works approximately 35 hours a week with the educator. Discussed Sanjay being required to complete 10 hours per year of professional development training. These certificates must be kept in a folder and available for review by the licensor.
Staff Records
First aid kit appears complete Band-aids, gauze, a CPR mouth shield, an instant cold pack, etc. Some of the emergency card pages for the children are not current. Discussed parents re-signing and re-dating the bottom of the pages if no information has changed.
Napping Materials
Pack and plays. Sleeping bags. Observed napping materials stored touching stacked on top of each other. Discussed storing these items in individual plastic bags.

Educator does not have the required medication parental consent forms or Individual Health Care Plan form. Allergy list must also be posted with this child's information and the location of the EpiPen. The EpiPen must be within easy reach of the educator, but out of reach of the children.
Fireplace in the basement playroom is blocked off and hearth is padded all around. Large television in one of the bedrooms is not secured to furniture or the wall and could pose a tipping hazard.

Indoor Space - free of hazards
Deck stairs were not gated at bottom. No gate was visible in the area. Hazards such as metal shovels, rakes, etc were accessible under the deck. Discussed replenishing mulch under the play structure that is used by children in the backyard.

Educator has not been tracking attendance since August 2024. The daily arrival and departure times of the children must be recorded. Attendance must be accurate at all times.
Progress reports for the children were not available for licensor review. Written progress reports are required every three months for infants and children with special needs, every six months for toddlers and preschoolers, and once a year for school aged children (5 years and older).
Children with [REDACTED]do not have the medication consent forms signed by the parents or the required Individual Health Care Plan filled in and signed by the pediatrician. These forms must be kept on file at the program. Allergy list is posted. Discussed placing a sheet of paper over the list to protect the children's privacy. [REDACTED] are brought back and forth from home in the children's backpacks. Discussed the program having [REDACTED] that stay in the program and located in the prescription box. [REDACTED] must be stored in a location where they are easily accessible to the educators but not accessible to the children.
[REDACTED] children's files were reviewed. Children enrolled are new to the program so all enrollment packets were completed in October when the program reopened. [REDACTED] is missing documentation of a current physical and immunizations. No physical on file at the program.
Exit sign is missing above the basement door to the garage. Emergency evacuation plan included in the parent handbook and posted.
