Mod Podge, which is toxic was observed stored in an unlocked cabinet within reach of children.
A review of one student's IHCP and allergy action plan called for diphenhydramine, however the educators were no aware of this.

The following was observed in the Fall Fives room: * The corner of the countertop has peeled and loose laminate. * The paint on the radiator cover is chipped.

* An educator in the toddler room was observed putting coloring pages and crayons on a table after snack time without cleaning and sanitizing the table first. * The toddler children were observed going directly to playing after snack without first washing their hands.
An expired package of "Go-gurts" yogurt was found in in the toddler refrigerator.
Sanitizing wipes were observed stored in an unlocked cabinet in Preschool Room 1.

Although the staff were knowledgeable about the Individual Health Care Plans, the following was noted through a random review of Individual Health Care plans: 1) One plan stated [REDACTED] does not provide the staff at the program specific information of when they should [REDACTED] . 2) One IHCP did not have the information of [REDACTED] .
At the time of the visit, children were observed using the outdoor climbing structures. The program's uses playground wood mulch as the impact absorbing material which does not act as intended when the ground is frozen. On the day of the visit the ground was not fully unfrozen.

Through a random review of three staff files, the following was noted: 1) One file did not have evidence of a current health physical 2) One file did not have evidence of a current professional qualifications registry certificate

A review of the fire drill log showed the last emergency evacuation drill was conducted in October.
A review of IHCPs showed one expired in March of 2022. Though the parent had resigned and dated it in May of 2022, the physician's is required to update as well.
