A staff member was observed washing their hands before cleaning and disinfecting the diaper changing table posing a risk of recontamination of their hands.

An educator was observed physically moving preschool age children who weren’t “listening” to them, standing over children and disciplining in and intimidating way and repeatedly saying “You’re not doing the right thing” without giving them direction on what the “right thing” is.
Through interview with staff, it was learned infant bottles are warmed in water that is warmed in the microwave and placed on top of the microwave where there is a risk of spilling and burning.

Preschool children were observed being allowed to use both climbing structures in the outdoor playground. One of the structures has "wood mulch" as the impact absorbing material which does not act as an impact absorbing material when the ground is frozen, as it was on the day on the visit.

An [REDACTED] Educator was observed changing Child A’s diaper on a mat in the classroom. The Educator did not have Child A wash his hands after this diapering change. The Educator also did not wash her hands after disinfectant the changing mat.

Diapering and Toileting for children under 2.9
[REDACTED] children who were napping in a gated off area of the classroom were not being adequately supervised as an Educator did not remain in the gated off napping area consistently while they were asleep. [REDACTED] children were under [REDACTED] , and were placed in [REDACTED] lined up behind other cribs. When all Educators were not within this gated area the children in the back cribs could not be adequately seen.
The Preschool Educators did not foster an self-independence opportunities for children when they did not allow the children to get their own coats/gloves/hats when getting ready to transition to outdoors. A child was observed walking to the coat rack to get her own belongings and she was told she could not do so. An Educator then stated that this child was new to the classroom and did not know the classroom rules.
Three boxes of Wheat Thins that were identified as intended snack food for the children in care had expired as of 12/15/2019
Although the Building Inspection was current, the program did not have a current fire inspection on file. The Director reported that the Fire Department came to the program on 1/26/2020 but has not yet received documentation of that visit, and the most recent inspection that was on file had expired.
The Educator did not follow the two step process of cleaning and disinfecting the diapering pad. The Educator only disinfected the diapering pad without first cleaning it with soap and water
