Random review of 6 staff files found: 1 without a current physical, 1 with evidence of only [REDACTED] and incomplete Essentials training for educator hired in 5/2024.
Attendance not accurate in one classroom. An educator had crossed out the time in a box and marked the time in a different box making it look like there was a different amount of children in the classroom than there was.

A child's emergency medication was not immediately available in the classroom the child was in. The medication was in the closing room from the day prior.

Review of 5 children's files found 2 files with enrollment forms dated in 2020, therefore they can't be considered valid.
A teacher who has been employed for longer than 6 months does not have current first aid on file.
When a teacher was asked if any children in the classroom had emergency medication she initially stated no. When emergency bag was checked, there were 3 children with emergency medications. Submit plan to ensure all staff are aware of all children with emergency medical plans.
Classroom bags holding emergency medication were left within reach of children in 2 classrooms. A child's epi pen and Benadryl were both expired.
One child's IHCP was expired as of 3/26/2022.
