There were hazards in the outdoor space. The fall zones under and around the climber did not contain the required depth of rubber mulch. It measured 4 inches in one area and 5 inches in another area. There were also plexiglass panels on the ground outside the playground gate, which posed a potential hazard to children.

It was learned that the emergency medication was left inside the classroom and not readily available when a group of 19 children were in the outdoor space.

It was observed in the Ants Room that the floors were broken and/or raised in several places which may cause a tripping hazard and the door leading to the bathroom was cracked along the bottom which may be a hazard to children.
A child's medication (Benadryl) listed on the Individual Health Care Plan was expired in the Ants Room.
