Licensor reviewed children's records and there were physicals and enrollment packets missing for two children. Please obtain all records and ensure all records are up to date; confirm once completed.
Blankets were stored together and not separate. Please separate blankets and ensure they are stored in a sanitary manner.
There were authorization for emergency treatment forms that were not signed. Please ensure all parents sign forms and add back into the first aid kit; confirm once completed.
There was a swing set, see saw, and little climbing structure in the outdoor play space that was accessible to children. Please make all structures inaccessible to children and upload pictures as confirmation.
