During an inspection of the outdoor space, it was determined that the bark mulch was compressed on the toddler and preschool playgrounds in the fall zones under and around climbing equipment, and the fall zones did not contain 9 inches of impact absorbing material.
It was observed that preschoolers use a door near the kitchen to exit to the playground, and the area does not contain a barrier to the kitchen, which contains hazards.

A bottle warmer is used in the infant room to heat bottles.
It was observed that there were hazards in the outdoor space. The wood chips in the fall zones under and around the climbers did not meet the 9 inch requirement and the climbers were not 6 feet apart.
The program does not have emergency medication (an inhaler) readily available for a child with a chronic medical condition.
