Licensor reviewed children's records and they were missing up to date physicals, lead tests and updated permissions. Please utilize the Children's Record Checklist and review all children's records to ensure they are complete; please confirm once completed and ensure that all records are maintained up to date moving forward.
Progress reports were not available for Licensor to review during this visit. Please ensure all progress reports are updated, and ensure a physical copy is kept in children's records.

The first aid kit was missing emergency information for all children. Please obtain these up to date forms and add to first aid kit; confirm once completed.

There continues to be chipping/peeling paint located on the upper trim of the home. Please ensure all chipping/peeling paint is removed and confirm once completed; a follow up visit will be scheduled.
Licensor walked through the indoor space and the following hazards were observed: There was an excessive amount of toys and items on the floor that created a tripping hazard. Please ensure floors are cleared and upload a picture once completed. The safety lock under the sink was broken making cleaning products accessible. Please add new locks to the kitchen and confirm once completed. Licensor walked through the outdoor space, and no hazards were observed.
Childrens Records - R Apr 2025
Refrigerator thermometer could not be located during this visit. Please add thermometer to refrigerator and confirm temperature was set to 32 and 40F degrees.
Licensor walked through indoor space and the following hazards were observed: There was an uncovered trashcan in the kitchen, accessible body wash in bathroom, and knives and scissors on kitchen counter top. Please remove all hazards and ensure moving forward that these hazards are not accessible to children; confirm once completed. Licensor wishes to license her two children's bedrooms and the hazards observed in the room was uncovered outlets. and there is also a bunk bed in the room. Please add covers to outlets and add a barrier to the stairs to ensure children's can't climb to the top bunk; upload pictures. Licensor walked through outdoor space was the following: There was a slide that exceeded 18 inches and uncovered grill. There was also a water table that was full of water. Please ensure slide is removed, all materials are emptied of water when not in use, and grill is covered when not in use. There was also access to a play structure that is located in the front yard. These structures cannot be accessible to children; please make accessible (removing it completely or making slide/swings inaccessible); upload pictures once completed.
There was chipping paint located on side and top of house near window sills. Please ensure all chipping paint is removed and confirm once completed; a follow up visit will be scheduled. Additionally, please upload the signed Peeling Paint Statement.
