Current consent for administered medications
The fall zones did not contain the required depth of impact absorbing material under and around the climbing structures in the outdoor space. The Licensor measured in four areas; the depth ranged from 2 to 5 inches, instead of the required 6 inches of rubber mulch.
One medication did not have written parental consent, as required.

The impact absorbing material under and around the climbing structure was inadequate. The rubber mulch measured 3-4 inches, instead of the required 6 inches.

During an inspection of the playground, it was found that the depth of the impact absorbing material under and around the swings and climbing structure does not meet the 9-inch requirement, and that there was a rusty tonka truck that poses a safety hazard for children.

Medication
During an inspection of the emergency medications, it was found that one Epi-pen was expired and Benadryl tablets were not in the original container.

During the visit, it was found that fire drills were not conducted in February, March, or April. 7.11(07) states that the educator must hold practice evacuation drills with all groups of children and all educators from each floor level of the approved space at least monthly. Drills must be held during different times of the program day, and must use alternative exits. The educator must document the date, time, exit route used, number of children evacuated and effectiveness of each drill.
