At the time of the visit one child with emergency medication was reviewed and the following was found: one emergency medication did not have a consent form completed for it.

In the Tropical Fish classroom, there was chipping and peeling paint on the bathroom vanity.

Feeding schedule for each infant/toddler; Infants held while fed from bottle
It was observed that in the Minnows classroom (Infant 2), an infant not held by a staff member while being fed a bottle. The infant was laying on a Boppy pillow drinking a bottle; potential cause for a choking hazard.
A review of six children’s files indicated: -Three did not have completed Individual Health Care Plans (IHCP).
In the hallway, between the infant and preschool rooms, chemicals were accessible to children including paint, eight cleaning cleansers, two carpet shampoos, and one can of aerosol.

Safe Sleep
The following was observed and accessible to children: HALLWAY -Outside of the Pre-K room leaning against the wall, a vacuum and mop were not secure. INFANT 2 -The diaper changing pad was torn. PRESCHOOL -In the bathroom, the step stool at the sink was soiled. -In the bathroom, there was dirt on the baseboards and sink.
Emergency Evacuation - R Apr 2025
On the day of the visit, a infant was observed sleeping in a Baby Merlin's Magic Sleepsuit. Program's serving infants must ensure that cribs, portacribs, playpens or bassinets used for sleeping infants under the age of 12 months do not contain pillows, comforters, stuffed animals or other soft, padded materials.
In Infant Room 1, there were 3 children in the classroom, but 2 were marked as present on the attendance list.
-On the day of the visit in Infant Room 1, one emergency evacuation crib did not have a clear evacuation path. -A review of the practice evacuation drill documentation indicated the program did not conduct drills in March, June, August, October, or December.
