An (REDACTED) infant was sleeping with two blankets and a towel. A (REDACTED) was sleeping on the second floor of the home. The educator stated the child had been napping on the second floor being supervised by the educator's (REDACTED). The educator denied visually observing the child at least every 15 minutes.
