During the visit, the attendance was inaccurate in the following classrooms. Three children were observed in the Infant/Toddler classroom. However, there were four children signed in on the classroom attendance form. Eight children were observed in the Toddler classroom. However, there were only seven children signed in on the classroom attendance form. The preschool teacher did not take their classroom attendance with them to the playground.
When the EEC licensor arrived at the program, they pressed the doorbell located at the entrance twice. No one answered the door to allow the licensor entrance to the program. An educator was supervising a group of preschool children outside on the playground. This educator stated that the Program Director was over at the new program getting things in order, and all other staff were covering in a classroom. The licensor stayed with the preschool group until they finished their scheduled outdoor time and re-entered the building. Previously, on 5/29/25, a monitoring visit was conducted. At that visit, no one answered the door to allow the licensor entrance to the program. The licensor called the program and was unable to speak with anyone. The licensor waited at the front entrance until an educator returned from break and allowed the licensor entrance to the program. At that time, all staff were covering in a classroom. The Program Director must be on the premises full time during program hours of operation to oversee daily administrative functions. EEC may visit an EEC licensed or approved program at any reasonable time to ensure that the program is in compliance with the law and with EEC regulations.

Three staff files were reviewed. The Licensee qualified a staff member as a preschool teacher under the Minimum Hiring Requirements. However, the staff member's file did not include evidence of work experience.
The Licensee did not follow Developmental Placement requirements. A review of one child's file indicated that this child was on a Developmental Placement Plan and began attending the Toddler classroom, which was outside of their chronological age on 3/10/2025. It was identified on the placement form that this child would be on this plan until 5/1/2025. As part of this process, monthly reviews of the child's adjustments must be documented and maintained in the child's file. Only one documented review (3/10/25) was maintained in the file.
In the Toddler classroom, 3 push pins were observed holding up postings and 1 push pin was used to hold up a sign with a sea horse on it. Push pins should not be used in classrooms with young children due to the potential for serious injury and choking hazards. This was previously cited on 5/29/2025.
The preschool teacher did not take a first aid kit with them to the playground. All staff working in the program at that time were covering in a classroom and would not have been able to bring a first aid kit outside to the teacher if a child was injured.

One child's Individual Health Care Plan signed by their physician expired on 6/14/2025.

The Toddler classroom attendance was inaccurate. There were 9 children observed in the classroom. However, only 7 children were signed into the room. This was corrected during the visit.
A sippy cup was not labeled with the child's name in the Infant/Toddler and Toddler Classrooms. Personal items intended for individual use by children, including but not limited to bottles, pacifiers, and sleeping materials, must be labeled with the name of the child for whom they are intended.
The following hazards were observed in the Toddler Classroom: Plastic pouches of diaper wipes and a plastic bag of diapers were observed on a shelf under the changing table and accessible to the children. Push pins were used in the classroom to hang postings and notices. Push pins should not be used in classrooms with young children due to the potential for serious injury and choking hazards.
