Three staff records were randomly selected for review, and the following was learned: 1) One Staff needed evidence of [REDACTED[as required by regulation. 2) Two Staff needed evidence of verification of two references.
The attendance in the Pre-K2 Classroom was not accurate. There were 13 children signed in, and 14 children were present.
This investigation inspection recorded no violations or advisories.

An electrical outlet in a classroom did not have a covering that prevents access to the receptacle openings.
There was an expired [REDACTED] in the medication bag. An [REDACTED] was not in the container in which it was initially dispensed.
A cabinet storing cleaning supplies was unlocked, leaving toxic substances accessible to children.
A child's IHCP did not have a parent's signature. Another child did not have a medication consent form signed.

During the medication review, the following was found: Two children were missing medication required on the child's IHCP in the program.
Fall zones adequate depth of impact absorbing material
Through a review of the [REDACTED]staff files the following was noted: 1)There was not evidence of [REDACTED] staff member of the date of hire, a date of orientation to the program. All staff files did not have evidence that current bi-monthly observations were being completed consistently.
At the time of the visit, both pre-k classrooms who were outdoors did not have a documented method of knowing where every child was at any give time, as they did not bring their attendance record outdoors with them.
The fire inspection and building inspection were not current.
At the time of the visit, the PWS was not current and expired as of 12/31/18.
