1. One student'[REDACTED] expired in November 2024. 2. A dose of [REDACTED] was found on site for a student without a prescription label or medical authorization for the program to use it. The student's IHCP created by the physician did not indicate giving this medicine, only to call an ambulance. The site coordinator stated the parents provided it to give to the paramedics should the student have a [REDACTED] The program should either return the medication to the parent or have documentation specifically stating why the medication is a the program.
A review of two staff files showed they were missing bi-monthly observations, annual reviews, and one file was missing evidence of orientation.

Children were observed arriving at the program and going to directly to eating snack without first washing their hands.

The program director was reported to have left the program in November. The educator in charge did not have a staff file and was not able to provide proof of qualifications.
Through an interview with staff, it was learned emergency evacuation drills were not being conducted.
Three emergency medications were observed on-site without the required medication consent.
There was no IHCP for a student who has severe allergies and asthma.
The following was observed: * Cleaning chemicals were observed withing reach of children. (This was cited at the previous visit) * The custodial closet was open allowing access to several toxic chemicals and hazards.
Toxic cleaning supplies were observed stored in reach of children.
A review of required postings showed the staff schedule was not posted.
A review of the fire drill log showed they are not being conducted on a monthly basis.
