The outdoor space that the toddlers use have several tripping hazards. There is a pipe that is a tripping hazard and the pipe is covered by a loose outdoor carpet and then there are areas of the outdoor carpet that are bunched causing a potential tripping hazard to children and staff.

Attendance in the preschool class was not accurate at the time of the visit. There were 12 signed in but 13 in attendance.

On the day of the visit, the licensor observed a teacher engage with a child in a negative manner. The teacher appeared to be frustrated with the child when she said to the child's sibling “I can’t understand what he is saying, do you know". This interaction was in front of the child and other children.
On the day of the visit, one child with a chronic health condition did not have an Individual Heath Care Plan (IHCP). In addition another child’s IHCP did not contain all the required information.
During the visit the following items were identified as classroom issues that address safety, cleanliness, and overall good repair: 1) The counter in the infant/toddler room has a seam between the top and side that is coming apart. 2)Areas of the walls in the infant/toddler room t/toddler room where the walls are dirty. 3) In the infant/toddler room there is a corner wall that has peeling sheet rock. 4) There is exposed sheet rock on the wall in the toddler bathroom 5)The preschool classroom had clutter on the shelves in front part of the classroom. 6) The shelve in the preschool classroom that stores the children's toothbrushes, had other items touching the toothbrushes. 7) There was a large plastic bag holding a child's belonging, that was on the floor, accessible to children.
