There is a deck that the children will not be using but the stairs need to be inaccessible to children. Please use a gate or a barricade to prevent children from climbing the stairs. Send a picture with the correction.

There was a trampoline and a wooden playscape in the yard that is used by the providers own children that needs to be inaccessible to children. Provider has three slides that are more than 18 in. high and they require a fall zone. Please remove the slides and make sure the trampoline and playscape are inaccessible.
One parent did not have their child tested for lead and there was a note in the child's file that said that the parent did not want to be screened for lead. Discussed with the provider that children should have a lead screening when they are old enough to do so. Provider is discussing this with the parent. Please provide an email stating that the child had the lead screen.
Provider is missing her medical form from EEC and trainings.
Provider is missing her CPR and First Aid card. Provider had a letter stating that she took CPR and First Aid. Provider stated that she would take it over again.
Provider is missing a first aid kit. Please show me a picture with all of the first aid components.
Provider has a pellet stove that is used during daycare hours. It needs to have a steady barricade. The barricade needs to be three feet from the stove as well as three feet high. Provider stated that additional gate pieces are ordered to provide the proper barricade. Please show a picture with proper measurements.
There was a gas tank in the yard that has to be inaccessible to children. There was a few spots of dog waste that needs to be inaccessible to children. Please remove the items.
Provider is missing her certificate since she was closed during Covid. Provider is in renewal so she will receive a new license after the renewal is complete. Please complete all of the non-compliances to that you can receive your new license.
Provider was missing an emergency back-up person to add to her required postings. Please put down someone that can help in an emergency.
PQR is not up to date and it is expired. Please up date the PQR.
