During the review of children’s files a child’s file was missing an IHCP (Individual Health Care Plan).
The posted allergy list needs to be updated, children need to be removed from list and then the children who have allergies need to be added to the list.

There was one EpiPen that did not have a prescription label affixed to it, another EpiPen was not in the box, but the script label was affixed to the EpiPens, but you could not see an expiration date, often that information is on the box.
Fire inspection is required quarterly for this program and was expired at the time of the visit.
During the visit the following was observed: There was clutter throughout the classroom (on shelves and counter spaces) and clutter in the entryway to the program. There were insulated cups on the counter, hot beverages can't be in classrooms as they could be a potential hazard. There was an empty fish tank on a shelf that could become a hazard to children if it falls and breaks. The shelf that this tank was on was secured on one side, but the other side was loose. There were plants on the ledge of a high window that could fall on children. There were several cords that were longer than six inches accessible to the children.

During the staff records review, it was learned that there was not evidence that staff received an annual review and consistent staff observations that occur once every other month, while the educator is working with children.
