Attendance in the Pre-K classroom was not accurate; 7 children were present and only 6 children were signed in.
When observing in the Pre-K classroom the following was noted: 1. The faucet to the bathroom sink was beginning to chip and rust.

Documentation of staff meetings were not on file every month, October and November 2023 were missing.
When observing in classrooms the following was noted: 1. Shelving units in the Older Toddler classroom were tippable as the Velcro was not completely attached. 2. Edges to the area rugs in the Pre-K classroom were frayed and lifted posing a potential tripping hazard.
This investigation inspection recorded no violations or advisories.
At the time of the visit the following was noted: 1. Plastic Step 2 kitchen dramatic play sets were not secured in the PS and PK rooms.

A bottle intended for an infant was not labeled with the child's name.
When observing the playground the following was noted: 1. the step leading from the inside to the outside is too high, posing a hazard. 2. 2 plastic round ottoman/seats were broken with sharp edges protruding. 3. Black weed control fabric was found exposed, this is a potential tripping hazard.
The program is moving, the wooden shelving units and dramatic play sets at the new location were not secured and were potentially tippable.

When reviewing the evacuation drill log it was learned drills were not being completed monthly as required.
Opened snacks such as saltines and Cheerios were found not stored in a clean/covered container in the Older Toddler room.
Swiffer Wet Jet floor cleaner was found stored on the floor next to the toilet in the Pre-K bathroom.
Plastic shopping bags were found hanging off of a shelf in the PS children's bathroom.
When observing a diaper change in the Older Infant classroom the following was noted: 1. The Educator did not wash the child's hands after diapering. 2. The Educator did not wash her hands after diapering. 3. The Educator did not wash the changing table pad with soap and water prior to washing it with bleach and water.
