3 children's files were reviewed and 1 file did not have a completed developmental history and it was missing the parent signature/date.
At the time of the visit some snack items were opened and not in a sealed container.
When observing in classrooms the following was noted: 1. The cover to the trash was dirty and in need of cleaning. 2. The dustpans that attach to the brooms were dirty and in need of cleaning.
The Geo dome did not have the 6-foot use/fall zone completely surrounding it as required.
At the pre-licensing visit to move the program to its new location the following was noted and must be corrected: 1. toilet bowl plungers and brushes were found in bathroom stalls. 2. Green shelf in the bathroom was not secured. 3. There were no screens on windows that were open. 4. Fencing does not reach the ground in many spots. 5. 2 ladders were outside leaning against the building. 6. The outdoor stairs in the back of the building leading to the basement were not secured and children could potentially fall down into the "hole", a wall needs to be built surrounding the opening or this area must be blocked off, so children do not have access to it.
Equipment is clean, safe, secured, and poses no hazard.
Electric outlets covered, no frayed electrical cords. Carbon monoxide detector.

At the pre-licensing visit to move the program to its new location the following was noted and must be corrected: electrical outlets were not covered with safety plugs.

Attendance was not accurate in the Panda classroom; 16 children were present and only 15 were signed in.

At the time of the visit the blankets in the Koala classroom were stored in a shelving unit touching.
Allergies were posted however the emergency medication was not listed on the posting as required.

When observing in classrooms the following was noted: 1. A lego table had a broken square with sharp edges. 2. Rest mats were torn with the inner foam exposed.
One child's [REDACTED]prescription label stated to discard after 12/28/2022.

This renewal - monitoring visit inspection recorded no violations or advisories.
