6 staff files were reviewed, and the following was noted: 1. 3 files were missing evidence of annual evaluations. 2. All files were missing evidence of observations and consultations every other month. 3. 4 files were missing evidence of the required amount of professional development hours yearly.

One hand on child when being changed on elevated surface
Yearly evaluations were not completed on staff as required.
6 children's files were reviewed, and the following was noted: 1. Date of admission was missing from 2 files. 2. 1 child was missing evidence of a [REDACTED] 3. 1 child was missing evidence of an updated physical exam. 4. 1 child's entire file of paperwork expired 12/23 and they were missing evidence of an updated [REDACTED]
The door to the kitchen was not closed and there was bleach stored on a low shelf accessible to children.
Clementines were found spoiled in a bin in the kitchen area.
When observing in classrooms the following was noted: 1. 1 child with asthma was transitioning into the Pre-K classroom his emergency medication was not with him; it was left in the PS room.
An Educator in the Toddler 4 classroom took both hands off of a child that she was diapering on an elevated surface.

Infection Control/Hand Washing - R Apr 2025
When observing in the Toddler 4 classroom the following was noted: 1. An Educator changed a child's diaper and did not wash the child's hands after. 2. An Educator did not wash her hands after changing the child's diaper. 3. The Educator washed the changing table with soap/water only, she did not disinfect the table as required.
Medication
1 child's prescription for an [REDACTED] stated to discard after 11/22/22.

Infection Control/Hand Washing - R Apr 2025
1. Chipping/Peeling paint was observed on a low window accessible to the children in the Infant 2 classroom. 2. A plastic slide over 18 inches tall was found in the Toddler gross motor room; there was not a 6-foot use/fall zone completely surrounding the slide and there was no impact absorbing material to protect from the fall height of the platform.
When observing in the Infant 2 classroom the following was noted: 1. The child's hands were not washed with soap/water after diapering. 2. The chanigng table surface was only washed with a sanitizer; it must be washed with soap/water and then disinfected.

A radio was placed on the windowsill with the cord running down the wall to the plug, the cord was not secured to the wall, posing a potential hazard.

When observing in classrooms the following was noted: 1, Sunscreen was found in a bucket inside a cubby accessible to children. 2. Swiffer wet jet with chemicals attached was found stored in a children's bathroom accessible to children
