Infection Control/Hand Washing
When observing in the program the following was noted: 1. Edges to area rugs in the second-floor hallway were curled up, posing a potential tripping hazard.
When observing in the Toddler classroom the Licensor noted the Educator cleaned the placemats and tables with a Clorox wipe and then sprayed it with water and wiped it dry. All surfaces must be sprayed with a soap/water solution to clean then wiped with a sanitizing wipe and the surface must remain wet according to the manufacturer's directions to sanitize. 2. An Educator did not wash the child's hands after changing the child's diaper. 3. An Educator did not wash her hands after changing one child's diaper, she just put on a new set of gloves and changed the next child.

At the time of the visit diaper creams were accessible to children in the downstairs bathroom off of the gross motor room and upstairs PS bathroom.
This investigation inspection recorded no violations or advisories.

3 Children's files were reviewed, and the following was noted: 1. 2 files did not have evidence of a physical exam. 2. 1 file did not have evidence of an updated lead screening (3 year old).
When reviewing the evacuation log it was determined the program is not practicing evacuation drills monthly as required.
At the time of the visit the Staff Schedule for all classrooms was not posted

Force of Nature spray was found accessible to children in the Preschool classroom. The Licensee removed it immediately when brought to her attention.
Of the 3 staff files reviewed the following was noted: 1. 3 did not have evidence of immunity to [REDACTED] or evidence of a program orientation.

Toxic Substances
1 staff member was hired 9/26/22 without having been deemed Provisional or Suitable by the Background Check Unit. The Licensee was told the Educator must leave the program until she has been deemed Provisional or Suitable by the BRC unit.
2 staff files were reviewed, and the following was noted: 1. 2 files did not have evidence of registration in the PQ registry. 2. 1 file did not have evidence of a completed BRC. 3. 1 file did not have evidence of a physical exam and immunity to [REDACTED] 4. 1 file did not have evidence of the required EEC Essential trainings. 5. 2 files did not have evidence of professional development trainings. 6. 2 files did not have evidence of observations and consultations every other month with the Educators.
1 Child's IHCP stated to administer [REDACTED] as part of their IHCP and there was none on site.
When observing in the Toddler classroom (Room 1) the Licensor noted diaper creams, sunscreen and lotions in a low shelving unit accessible to children in the child's bathroom.. 2. Educator backpacks/purses were found stored on the floor of the Toddler room accessible to children.

The wall was moved over so they now have the 6 foot use zone surrounding the climber however; they do not have the impact absorbing tiles in the 6 foot use zone completely surrounding the climber.
