Licensor randomly reviewed 4 of the children's progress reports. Of these, 2 were complete and up to date, but the other 2 were out of date.

Licensor reviewed 4 of the children's records. None of these records were completely up to date. There were 3 records that had out of date medical information, including physicals and immunizations, and the 4th record had no evidence of lead testing.
Educator has not completed the EEC Essentials 2.0 training.
The 1st aid kit was missing an instant cold pack and had out of date emergency contact forms for the children.
Educator completed a total of 25.5 hours of professional development over the last 3 years. She is missing 4.5 hours.

Observations from original visit on 2025-01-27 00:00:00: Educator has a child in the home who turned 15 years old. This child is not listed as a household member and has not gone through the BRC process. Corrective Plan received on 2025-04-12 00:00:00: Jonathan will be added at the end of may. **UPDATE ON 4/17/25:** Educator still has not added her 15 year old son as a household member or started the BRC process for him.

Observations from original visit on 2025-01-27 00:00:00: Educator was not exceeding her licensed capacity during the visit. There were 3 children present, ages 12 months, 8 months, and 4 years. However, there are currently 4 children enrolled under the age of 2 and only 1 of those children is over 15 months. There are multiple days during the week when all 4 of these children are all present. Discussed the regulations around capacity and that there can never be more than 3 children under the age of 2 years present and 1 of those children has to be at least 15 months old and walking independently. Corrective Plan received on 2025-04-12 00:00:00: Talked with the parents about kids days in the daycare and rescheduled accordingly **UPDATE ON 4/17/25:** There were 4 children present, ages 11 months, 13 months, 19 months, 22 months. All 4 children were under the age of 2 years. Discussed with Educator again that this means she exceeded her capacity.

The following hazards were observed: **LIVINGROOM** - outlets exposed on surge protector by the TV **DININGROOM** - outlets not covered **BASEMENT: PLAYROOM** - no hazards **NAPROOM** - outlet behind pack and play not covered **LUNCHROOM**- no hazards **BATHROOM** - hygiene products in shower and on sink **Staircase** - cleaning products on stairs **OUTSIDE** - fall zone still does not extend 6 ft in every direction.
Persons on Premises
Educator installed a new play set in the outdoor playspace and there is no fall zone. There is a little bit of mulch underneath parts of the set, but not to the extent that is required. Educator will need to install a fall zone that extends 6 feet out in every direction and is a 9 feet depth. The set is also too close to the fence (50 inches) and will need to be moved as the fallzone will need to extend 6 feet.

There is 1 child enrolled in the program who requires a progress report. This child's report was overdue.
Licensor inspected all of the licensed, indoor space during the monitoring visit. The only indoor hazard that was observed was in the basement playroom. The play kitchen is a tipping hazard and needs to be anchored to the wall. In the outdoor playspace, Educator installed a new, large playset, and did not install a fallzone. Also, the grill did not have a cover.

The most recent fire drill was conducted on 1/13/23. The Evacuation Log for 2022 was missing some information. There were 6 months that did not have the exit used written in and 3 months did not have the time of the drill entered.

• Licensor discussed with Educator that she must remain on the same floor level as the childcare children at all times. During the renewal visit, Licensor observed Educator leave children on the basement level on 2 separate occasions, when she went to the first level to greet people entering the program.

Licensor reviewed 2 of the children's progress reports and they were both overdue. Please ensure that all enrolled children have a complete and up to date progress report.
CPR and First Aid expired 03/2021, a year ago. Educator stated she is signed up already for a course this weekend.
During the monitoring visit, Licensor witnessed Educator go to the first floor while leaving the children in the basement space unsupervised, on 3 separate occasions. When this was addressed with Educator, she stated this is a common occurrence, especially when the children are arriving in the morning. Educator stated she ensures the children are in high chairs when she goes upstairs. Licensor explained that regulations state that Educator must remain on the same floor as the childcare children at all times.

Educator discussed with Licensor that she did not complete fire drills every month in the fall of 2020, after she reopened. She felt it didn't make sense to do this because she had only one child. Educator stated she has been completing the fire drills recently. However, this was not documented anywhere in the program. Licensor discussed that fire drills need to be completed and documented every month, regardless of how many children are present.

Educator could not locate her Evacuation log during the renewal visit. Please upload the fire drill log once it is located. Please ensure that a fire drill is being completed at least once a month and then documented.
Educator has a changing table with a changing pad, and Educator uses a disposable layer (paper towels). Licensor observed Educator change an infant’s diaper during the renewal visit. Afterwards, Educator did not wash her hands or the infant’s hands. There is also a cover on the changing pad that is cloth and needs to be removed as it can not be disinfected after each diaper change.

Licensor reviewed all 4 of the children's records. Of these, two children were overdue for progress reports, one child just started and didn't need a progress report, and one child had a complete and up to date report. Please ensure that all children have a complete and up to date progress report.

The first aid kit was missing 1 child’s emergency contact information. All of the other required items were observed.
During the visit, while Educator was speaking to Licensor, a 4 year old child went upstairs to the first floor to get her lunch box and then came back down stairs. Educator was not aware that the child had gone to a different level until Licensor informed her after seeing the child on the stairs.

Licensor reviewed 2 of the children's records. Both of these records were missing a current physical. One record also had out of signatures in the enrollment packet.
When Licensor arrived at the program, there was a cloth cover on the changing pad. This was removed during the visit. Educator is aware that she should not be using this cover during diapering. Educator also uses a disposable layer and she reports that she disinfects the changing pad after each use.
The water temperature was 130F degrees. Regulations state that the temperature must be 120F degrees or below.
As you exit the home into the backyard, there were a few hazards on the edges of the stairs, including cleaners and a can of paint, that were accessible to the children.
Educator completed 14 hours of professional development in 2022 and 5 hours in 2021. She did not have any training certificates from 2020. This totals 19 hours of professional development. The requirement is for Educators to complete 10 hours per year. This means that Educator is required to complete an additional 11 hours of training.

Licensor was able to inspect all of the licensed space in the home, as well as the outdoor playspace. The following hazards were observed: ** LIVINGROOM - exposed outlets in power strip, lots of wires next to the tv. ** DININGROOM - no hazards ** BASEMENT: PLAYROOM - no hazards ** NAPROOM - exposed outlets ** LUNCHROOM - no hazards ** BATHROOM - cleaners and hygiene products under the sink ** OUTSIDE - old furnace that was removed from the home is in the yard. Educator stated it will be removed soon, but it is currently a hazard.
Educator has been using the daily attestation forms that the parents sign as her attendance. Does not track the time of arrival and departure on a daily basis.
Educator does not have [REDACTED] with [REDACTED] . Licensor stated this information is required to be posted confidentially, with a cover sheet.
For diapering, Educator has a changing table with a changing pad. Educator has not been using a disposable layer, but has been disinfecting the changing pad after each use. Educator only has one child who is in diapers. Licensor explained to Educator that she does have to use a disposable layer.
Educator could not locate her training certificates during the renewal visit. Once these certificates are located, please upload them. Please ensure that 10 hours of professional development are being completed annually.

Educator reviewed all 4 of the enrolled children's records. Of these, 2 were complete and up to date, and the other [REDACTED] had out of date [REDACTED] information [REDACTED]. Please ensure that all children's records are complete and up to date and also provide a plan as to how the records will remain up to date in the future.
Licensor inspected the first aid kit. Educator needs updated emergency contact forms for the children. The kit was also missing gauze pads.
Educator showed Licensor the emergency numbers in a binder. However, these numbers are not currently posted anywhere in the program.
Educator has a toothbrush for one child, but not the other children.
Educator informed Licensor that she does at times run up to the first floor to grab something while the children remain in the basement. Discussed that regulations state Educator must remain on the same floor level as the childcare children at all times.
Licensor inspected all of the licensed space and observed the following hazards: ** Outside - no hazards, fenced in back yard **Basement ** Nap room - no hazards ** Playroom - no hazards ** Lunchroom (add to License)- no hazards ** 1st Floor ** Livingroom - outlet on the floor was not covered ** Diningroom - no hazards ** Bathroom - hygiene products in the shower need to be up at least 4 ft, or the shower door needs to remain closed but it was open during the visit, one drawer in the vanity has a child proof latch that is not working.
Educator has been using a room attached to the nap room that is not currently licensed. This room will be added to the licensed space at the end of this renewal process.
