Random review of 2 staff files found 2 with no evidence of a current physical and one with no evidence of [REDACTED]
Attendance was not accurately recorded in two classrooms. A toddler classroom had 9 children with 8 signed in and a preschool classroom had 14 children with 13 signed in.
Not all educators knew how to administer all emergency medications.
During health and safety inspection the following was found: not all refrigerators were clean, dirty dustpans accessible to children, a blue couch in PreK that needs to be replaced and broken tiles in hallway.
A child with febrile seizures was not listed on the allergy/emergency condition/medication posting.
Sunscreen was found stored in a container on top of a small refrigerator accessible to children in the Pre-K classroom.
When observing in classrooms the following was noted: 1. The Toddler 2 bathroom was full of items that did not belong in a bathroom and 2 children were in the process of potty training making it not easily accessible. 2. 2 small dustpans in the Pre-K classroom were dirty and had food residue in them.

At the time of the visit the following was noted: 1. Brooms and a dustpan was found leaning against the children's cubbies in the Toddler 2 classroom. 2. There were no paper towels in the Pre-K/ PS bathroom, a roll of paper towels was found stored on the table in the Pre-K classroom, this is not sanitary. 3. The dramatic play clothing rack was not secured and potentially tippable in the Toddler 2 classroom. Observations from original visit on 2023-05-10 00:00:00: When observing in classrooms the following was noted: 1. Swiffer wet jets were found accessible to children. 2. Brooms were found leaning against walls and not secured. 3. Paper towels were found stored on counter tops and not in the holder, this is not sanitary. 4. When observing in the PS classroom the Licensor noted an Educator grabbed a child by the upper arm and lifted the child up off the floor to move the child over onto the rug. As the Licensor continued observing the Educator grabbed 2 additional children by the arm to move/guide them into a different spot on the rug.

When observing in classrooms the following was noted: 1. A Toddler 1 Educator lifted a crying child off of the floor to standing position by pulling the child's 2 hands. This same Educator stood behind the child who was seated at the table, took the child's sippy cup away from him and picked him up off the chair and brought him over to the sink without telling him what she was going to do before doing it. 2. 1 Educator in the Toddler 2 classroom was observed cleaning the room up and not interacting/engaging/playing with the children for more than 20 minutes.
When reviewing medication the following was noted: 1 child's medication consent form was not signed/dated by the parent. 2. 1 child's medication consent form had expired 4/11/25.
The Geo dome did not have the 6-foot use/fall zone completely surrounding it, it was too close to the fence.
When reviewing IHCP the following was noted: 1. 1 IHCP said to administer [REDACTED] and the parent brought in [REDACTED] 2. Several IHCP's were not signed/dated by the parent. 3. Several IHCP's did not list what Educators are trained in that child's IHCP/medications and the form did not list the side effects. 4. 1 Educator stated only 1 child with an IHCP/medication in the room and there were 3 children. 5. 1 child with [REDACTED] did not have an IHCP on file. 6. 1 child's IHCP form was missing a page, so all symptoms were not listed.
1 child's IHCP stated to administer Benadryl and there was no [REDACTED] on site the parent brought in [Redacted].

7 children's files were reviewed, and the following was noted: 1. 1 file had expired 3/2024. 2. 1 child's file was missing evidence of a physical exam.
8 staff files were reviewed and the following was noted: 1. 2 files were missing evidence of the verification of 2 verbal references. 2. 4 files were missing evidence of a yearly evaluation. 3. 1 file was missing evidence of a physical exam. 4. 1 file was missing evidence of updating in the PQ registry. 5. 1 file was missing evidence of the 12 EEC Essential trainings.
Children in the Toddler 4 classroom were coloring on photocopied worksheets.
When observing in classrooms the following was noted: 1. An adult chair was stored on top of a water table in the Toddler 1 classroom. 2. 2 Darth Vadar chairs were starting to tear/rip. 3. An air purifier was stored on top of a small refrigerator not secured posing a tipping hazard in the Toddler 2 classroom. 4. Children's wooden chairs were stacked up against a shelving unit posing a potential tipping hazard. 5. A Keurig coffee maker was in the Infant 1 classroom.
When observing in classrooms the following was noted: 1. There was not a sufficient amount of toys/ materials accessible to the children in the Toddler 1 classroom. 9 shelves were empty and there was no dramatic play food, clothes, pots/pans etc... 2. Children in the Toddler 1 and Toddler 4 classroom were given a choice of only 1 marker/crayon to color with and the remainder of the markers were taken away. 3. A shelving unit in the PS classroom was turned around making it not accessible to children.
Educators in the PS classroom were setting a timer and making children stay in 3 areas, the children were not allowed to change areas until the timer went off. Once child went over and took out the chalk board and was told that area is not open and was forced to go back to the table until the timer went off.
Written emergency evacuation procedures and diagram of escape routes were not posted by each potential exit out of the classrooms.

Observations from original visit on 2023-05-10 00:00:00: At the time of the visit the following was noted in the Toddler classrooms: 1. There was not a sufficient amount of toys/equipment/materials available to children, some shelves were empty. 2. Of the 3 children's files randomly reviewed the following was noted: 1. 1 child's file expired 9/21/2022, there was no evidence of a physical exam or lead screening. 2. 1 child's progress report was not dated so it could not be determined if the program was completing progress reports every 6 months as required. 3. 1 child's date of admission was missing from their file.
3 Staff files were randomly reviewed, and the following was noted: 1. Verification of 2 verbal references was missing, evidence of Teacher qualifications, evidence of a current physical exam, and the program orientation was not signed/dated. 2. Documentation of interview date was not recorded in the file, verification of 2 verbal references was missing, evidence of registration in the EEC PQ registry, evidence of having completed the 12 EEC essential trainings. 3. verification of 2 verbal references was missing.
Several staff members who have been employed longer than 6 months did not have evidence of current First Aid training.
At the time of the visit the following was noted in the Toddler classrooms: 1. There was not a sufficient amount of toys/equipment/materials available to children, some shelves were empty. 2. There was little to no dramatic play equipment in the housekeeping areas; plates, utensils, food, pots/pans, clothing etc.
At the time of the visit individual sippy cups, water bottles and bottles were found not labeled with the child's name for which they were intended.
At the time of the visit the kitchen refrigerator temperature was 15 degrees, for safe food storage it must be kept between 32-40 degrees and checked daily.
When observing in classrooms the following was noted: 1. Swiffer wet jets were found accessible to children. 2. Brooms were found leaning against walls and not secured. 3. Paper towels were found stored on counter tops and not in the holder, this is not sanitary. 4. An air purifier was found stored on top of a sensory table. 5. A toilet bowl plunger was found in a bathroom accessible to children.
Chipping/peeling paint and several small holes in the walls were found in the Toddler 1 classroom.
When reviewing IHCP's and medication the following was noted: 1. 1 child's IHCP was expired. 2. 3 children were listed on the allergy list as allergic to [REDACTED] and there was no IHCP's on file for these children.

At the time of the visit the following was noted: 1. Attendance was not accurate in 2 classrooms; The infant classroom had 5 children in attendance and only 4 children were signed in and the Toddler 2 classroom had 8 children in attendance and 11 children were signed in. 2. The program exceeded the maximum group size of 9 children in the Toddler 2 classroom; the attendance sheet in the Toddler 2 classroom stated 15 toddlers were present at one time, no children were signed out with a departure time as required.
At the time of the visit the following was noted: 1. There was a small hole in the wall in the Toddler- 2 classroom. 2. Window shade was torn/ripped and in need of replacement.
When observing in the PS classroom the Licensor noted a tall plastic teacher cabinet was not secured and potentially tippable.
