2 large, stained ceiling tiles were observed within the "program space" at the back of the building (just outside of the lab room), creating a potential hazard to children.

1) Throughout the visit, the program had a "difficult" time remaining in ratio. At several points, there were more than 26 children and two staff in the gym space. Additionally, the Assistant Site Coordinator left the grouping in the gym without a YMCA staff member present in order to assist another educator with a transportation issue. 2) On the day of the visit, staff had 31 children total in Room 11, which measures for 26 children total, the program exceeded the total capacity allowed for that classroom, creating a potential hazard.
During a review of the program space, the following was observed: 1) all area rugs were observed to be dirty, with stains and spills 2) educators purses and bags were observed to stored in the reach of children 3) the emergency medication bag was in reach of children, creating a potential hazard when it was observed to be stored on a bench outside 4) cleaning sprays and cleaning wipes were observed to be stored in the reach of children, creating a potential hazard 5) it could not be determined if food was being stored safety in the refrigerator as there was not a thermometer present as required 6) Educators drinks were left in the reach of children, creating a potential hazard.

During a review of the program's space, it was observed that 1) there are stained ceiling tiles throughout the program's space, including the children's bathroom space and the hallway 2) there is a stained ceiling tile in the hallway in front of the 2 back classrooms that is buckling and considered to be potential hazard.

Throughout the entire visit, it was observed that children were not properly supervised. Educators were not clear regarding which children were in their groups or how many children they were responsible for. While walking to the bathroom, Child C heard the sound of the vacuum and after covering their ears, attempted to close themselves off into the janitor closet, but the Licensor was able to intervene. A YMCA staff did not witness that incident, as Child C was sent to the bathroom alone, without staff supervising.
Program staff appeared "stressed" throughout the visit and made statement to children such as "we are short staffed today; we are short three educators. So today everyone has to listen" Another educator was observed to state to children more than once, "this line (referring to the children lined up to go outside) is nasty, we aren't every going outside with such as nasty line."
A staff scheduled was not posted as required by regulation.
While children were accessing the outdoor space, they were found to not be properly supervised when educators brought children outside, but allowed them to access the entire outside space, without enough staff to supervise. As a result, children were climbing into the pickle ball courts of adult players (not related to the YMCA) and accessing the infant equipment on the playground without being redirected to use equipment that is appropriate for school aged children.
1) Child A had a prescription on the [REDACTED] that stated it expired 8/1/2024 2) Child B had a current epi pen, however in the same emergency medication bag was their expired epi pen (expired 1/2024) creating a potential hazard.
1) Children were observed to use hand sanitizer to clean their hands before eating snack on the "stage" however, the bathroom with a sink space is located directly outside of the stage door. 2) Children were observed to be served snack both on the stage space and in the classroom, the tables were not cleaned and sanitized before children sat down to eat and they were allowed to eat all over the classroom, while they were walking through the gym space and as they were walking down the hallway, creating a potential hazard.
The disinfectant was observed to be stored in the reach of children when it was left on top of a toy shelf.
The road rugs in both classrooms were observed to be dirty. Children's belongings, including water bottles, back packs and other items were observed to be stored on the ground outside at the "camp site" and all over the floor inside in the classrooms, creating a tripping hazard.
On the day of the visit, written evacuation directions were not posted for "room 10" and there were not evacuation procedures and a map posted on the secondary door as required.
Emergency medication was observed to be stored in program backpacks, however, the backpacks were observed to be stored on tables outside and inside, allowing children access to the medication.
On the day of the visit, cleaning chemicals were observed to be stored in an unsecured cabinet, creating a potential hazard.
