1 Educator did not have a completed BRC prior to working with children. On 11/7/25 the BRC is still pending, the Educator was not deemed provisional or suitable as required prior to hiring. The staff records checklist stated an Educator began employment on 2/28/25. The BRC Navigator stated the Educator's BRC was entered into the system on 9/18/25.

3 staff files were reviewed and the following was noted: 1. 1 file was missing evidence of an updated physical exam hers expired 9/2024. 2. 1 file was missing evidence of competing the EEC Essential trainings.
3 children's files were reviewed; 1 PS age child file was missing the physical exam, [REDACTED].
At the time of the visit 2 SA children were allowed to leave the 2nd grade classroom and walk down to the bathrooms in the K/1 classroom unsupervised. The Educators were not aware of the 2 children's activities at all times in a public space and there was no communication that the children arrived safely to the K/1 classroom bathroom. The program was cited for this on 11/15/23, the corrective action plan submitted on 11/17/23 stating: Calls will be made to each room and children watched as walking from A to B was not being followed.

At the time of the visit 2 SA children were allowed to leave the K/1 classroom and walk down to the 2nd grade classroom unsupervised. The Educators were not aware of the 2 children's activities at all times in a public space and there was no communication that the children arrived safely to the 2nd grade room.
Supervision of Children
3 Staff files were reviewed and the following was noted: 1. 2 files were missing annual evaluations, 1 file was missing evidence of [REDACTED]and 1 file had evidence of immunity to just [REDACTED] as required.
When observing in the K/1 classroom the following was noted: 1. The Educators did not take attendance as the children arrived at the classroom, after 15 minutes, the Licensor brought it to the Educator's attention and then attendance was recorded. 2. Two 2nd grade children were in the K/1 classroom for lunch, however, they were not signed into the K/1 classroom's attendance sheet, so the attendance was not accurate. 3. 1 staff members child who typically does not attend on Wednesday's was not signed in on the attendance sheet causing the attendance to be inaccurate for several minutes until an Educator could figure out who was missing from the sheet. 4. Upon the first bus arrival 1 Educator was alone with 17 SA children until the next Educator arrived. 5. The program did not maintain Teacher: Child ratio when an Educator left 7 PS age children alone with the soccer coach.
Several bottles of hand sanitizer were found stored on low shelving units/tables accessible to children in all 3 classrooms.
When observing the playground the following was noted: 1. Green astro turf covering the stairs that lead to the playground is not secured on one stair, posing a potential tripping hazard. 2. Black landscaping fabric is protruding through the rubber mulch posing a potential tripping hazard around the climbing structure.

At the time of the visit the attendance was not accurate in the PS classroom and the 2nd grade classroom. 6 children were present in the PS room and only 4 were signed in on the attendance. 4 children were present in the 2nd grade room and only 3 were signed in on the attendance.

At the time of the visit [REDACTED] joined the K/1 group that was in the dance studio, however; the K/1 Educator did not sign [REDACTED] into [REDACTED] attendance sheet. The [REDACTED] attendance sheet did not have [REDACTED] to dance signed out. The [REDACTED]attendance sheet stated 23 children were signed in, however; only 21 children were present. [REDACTED] children were absent and they were not marked "out" until the Licensor brought it to the Educators attention.

At the time of the visit an Educator left 7 PS children alone/unsupervised with the soccer coach for approximately 45 minutes. The soccer coach did not have an EEC Background Record Check.
A sink used for hand washing after toileting was used to fill a child's water bottle with water.
At the time of the visit the following was noted: 1. SA children did not wash their hands prior to eating lunch.
