Outdoor Space
When observing the outdoor play space the following was noted: 1. A Railroad tie used on the playground to hold in mulch had holes on the top and the front of 1 railroad tie was deteriorated, 2 metal spikes were exposed posing a potential hazard. 2. A Little Tykes tall plastci slide was on the grassy area, there was no impact abosorbing material in the 6 foot use/fall zone as required.

6 children's files were reviewed transportation plans were missing from all files.
6 staff files were reviewed, and the following was noted: 1. 3 Educators were missing evidence of the verification of 1 verbal reference. 2. 1 Educator was missing evidence of the documentation of the interview date, the verification of 2 verbal references, evidence of updating in the PQ registry and evidence of professional development for the 23-24 school year. 3. 1 Educator was missing evidence of a program orientation. 4. 1 Educator was missing evidence of an updated physical exam. 5. All staff were missing evidence of observations and consultations every other month.
The evacuation drill log was missing evidence of practicing a drill in August 2023, December 2023 and April 2024.

When observing in the Chipmunk classroom the Licensor noted hand sanitizer and sunscreen stored on low shelves/window sills accessible to children.

Snack items such as goldfish were found rolled closed with tape, not in a sealed/covered container or ziplock bag.
1 child’s IHCP had expired 2/16/23.
Evacuation procedures posted at each exit
When observing the playground the following was noted: 1. Little Tykes plastic cube was within the swings 6 foot use zone on the left side of the poles. The cube must also have a 6 foot use/fall zone completely surrounding it. 2. Black benches were within the swings use/fall zone.

At the time of the visit written emergency evacuation procedures were not posted by the main door leading from the Butterfly room into the hall. The Director stated they are being retyped for all exits.
