During the inspection of the program, the following was found: 1) In the Owl Classroom, the multiple rest nap mats were stacked on top of a shelf and were tipping over, causing a safety hazard for children. 2) In the Owl Classroom, rust was found on multiple children's chairs. 3) The Garbage containers did not have a cover in the Owl Classroom. 4) Peeling/chipping paint was found on walls in both classrooms.

Hand sanitizer was found within the reach of children in the Chickadees Classroom.
During the IHCP/Medication Review, the following was found: 1) One child did not have evidence of a required medication listed on the IHCP. 2) One child was missing a medication consent form.
Two staff records were reviewed, and the following was found: 1) One staff member did not have evidence of the date/documentation of the interview. 2) One staff member did not have evidence of a staff form.

At the time of the visit, the following was found: 1) Chipping paint was found on the blue shelves in the Chickadees' classroom. 2) The baseboard in the children's bathroom was missing, exposing drywall.

Three children's records were reviewed, and the following was found: Three children's files did not have evidence of current updated paperwork.
During the Safety inspection of the program, the following was found: In the Owl classroom, peeling/chipping paint was found.
Two Staff Reports were randomly selected, the following was found: 1) Two staff member did not have evidence of verification of 2 references. 2) Two staff members do not have evidence of a program orientation. 3) One staff member did not have evidence of documentation of interview.
During the safety inspection of the playground, the following was found: 1. The chain link fence at the front of the driveway is damaged. 2. Multiple wires are hanging along the side of the building.

Notification of Change of Administrator
During the safety inspection of the inside of the program, the following was found: 1) Multiple shelves must be secured in the Owlets Classroom. 2) In Multiple areas of the Owl and Owlet Classroom, chipping/peeling paint was found.
During the playground inspection, the following was found: 1) A new dome climber was installed without the adequate depth of an impact-absorbing material underneath. The program cannot use the dome climber until an adequate depth of impact-absorbing material is installed.
The program did not notify EEC that there was a change of administrator at the location in July 2024.
